- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81K00-09-T-0253 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32. This acquisition is unrestricted under NAICS code 339113, Size STD 500. The requirements in this solicitation: 1. CHIROPRACTIC TABLE (9511101) GALAXY ULTIMATE COLOR CABERNET/CABERNET Qty 2 2. T-BAR (634100) QTY 2 3. COMPLETE SET OF SNAP COVERS (697420) QTY 2 4. OPERATOR/SERVICE MANUALS QTY 4 MFG: LLYOD TABLE COMPANY Vendors offering other than the brand name product shall meet or exceed the following Salient Characteristics: Galaxy Ultimate can be tilted or lowered hydraulically at infinitely adjustable speeds from horizontal to 70 degrees controlled by foot pedals on both sides of the table. A safety switch prevents the table accidently lowering from the hylo position. A foot plate automatically comes up to meet the patients feet when table begins to lift. Color: Cabernet/Cabernet. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Clause 52.211-6 (and other FAR clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, 30 days after receipt of order, shipping shall be FOB destination to Evans Army Community Hospital, Fort Carson, CO 80913. Acceptance Location: Evans Army Community Hospital, 7500 Cochrane Circle, Fort Carson, CO 80913. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program.
