- Attachment #1, Pricing Sheet, is to be used to submit proposal. Service Contract Act Wage Determination for Ohio Co., WV Statement of Work SF 33 (Sections A-M) Section L. Instructions for Submitting Proposals & Questions Section M. Evaluation Criteria and Basis for Award The U.S. Department of Labor, MSHA (Mine Safety & Health Administration) is seeking Firm Fixed Price proposals for grounds maintenance services in Triadelphia, West Virginia, located at 765 Technology Drive, Triadelphia, WV, 26059. Please carefully read the attached Statement of Work to understand the full scope of services that are being requested. Please provide a Firm Fixed Price proposal based on the attached Wage Determination No. WD 05-2573, Revision No. 7, Dated 6/2/2009, for Ohio County, West Virginia, including any fringe benefits you may offer. All terms and conditions of the Service Contract Act apply (i.e. health & welfare benefits). You must indicate how the benefits package is administered. *Pricing is subject to change at beginning of each Option Year based on current Wage Determination. Period of performance will be from award date of this contract and continuing for 12 (twelve) months with 4 (four) option years. Proposals must include Firm-Fixed pricing for a base and 4 (four) option year periods (Attachment #1). Proposals must be based on the quantities stated on Attachment #1 for evaluation purposes; however, the government will only pay for the services actually rendered. For example, the number of grass cuttings needed will depend on rainfall amounts; therefore, the government may not need the total number of grass cuttings listed on the quotation sheet and will only pay for the actual number of cuttings performed. All option years are subject to availability of funding. Proposals that do not include base and all option years will not be considered. Offerors are required to provide a technical and price proposal and 3 (three) past performance references for contracts that are similar to the work to be performed. The successful offeror will be required to submit a written offer signed by a corporate officer with authority to commit the company to performance. Award of this contract will be based on Best Value for the Government, pricing and other factors considered. A site visit is scheduled for Monday, July 20, 2009 at 10:00 a.m. EST. at the Triadelphia, WV location. Attendance of the site visit is mandatory in order to appreciate the size and layout of the facility to fully assess the needs of the Government. The Government will not provide reimbursement of costs incurred by vendors for the site visit. Point of contact for the site visit is David Diegmiller, phone (304)547-2041.
