- COVER LETTER, SF18, STATEMENT OF WORK, RFQ CLAUSES AND EVALUATION CRITERIA. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued using simplified acquisition procedures 13.5. This solicitation is a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through federal acquisition circular 2005-34. This solicitation is a 100% Small Business Set Aside. The associated North American Industrial Classification System (NAICS) code for this procurement is 624190 Counseling Services (Substance Abuse). Offerors must submit Quote in response to solicitation CSOSA-09-R-0021 prior to July 22, 2009 at 3:00 P.M. EST (local). Electronic versions of the offeror’s quote will not be accepted. Offerors may submit quote via overnight express, by courier, in person delivery or by U.S. Postal Services.
