Description
DEPARTMENT OF VETERANS AFFAIRS (VA) IS CONDUCTING A MARKET SURVEY AND DESIRES TO LEASE OFFICE AND MEDICALLY RELATED SPACE FOR THE OCCUPANCY OF A MENTAL HEALTH CLINIC. VA will also consider acceptance of donated space. The offered site must contain between 7,000-9,999 square feet and have adequate parking, including handicapped parking as required by law. The property shall be located within the following delineated area in the city of Jacksonville, Florida: West boundary Interstate 295; East Boundary 9A, North Boundary 295-North, South Boundary 295 South and also including the entire town of Orange Park FL. To be considered, the site must be located within the delineated area, be on or near public transportation, have adequate on-site parking for employees and veteran beneficiaries, and be handicapped accessible. Zoning must be appropriate for operation of a mental health clinic. Sites that are easily accessible by major roads and public transportation requiring minimal build-out are most desirable. Sites previously used for a Medical Occupancy are preferred. Sites requiring extensive build out will not be considered due to the urgency to occuppy the space. Offerors must provide evidence that offered properties are outside the 100-year flood plain and that there are no environmental issues associated with the property. This includes wetlands designation, hazardous waste issues, etc. VA staff will conduct a market survey of offered properties. Interested offerors (owners, brokers, or legal representatives) shall submit one copy of the property description including a floor plan of the space and photo or drawing of the site to Susan Little by mail, fax, or e-mail no later than 4:00pm by July 15, 2009, at the following address: Department of Veteran Affairs, VA Medical Center, ATTN: Contracting Office (90C), 619 S.
