Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Quotes (RFQ) #246-09-Q-0032. This procurement is a 100% small business set-aside under NAICS code 561320 with a standard business size of $13.5 million. Contractor shall provide a firm-fixed hourly rate (all inclusive to include travel and per diem) for 520 hours, August 17, 2009 to November 13, 2009. Contractor will work forty (40) hours per week. Estimated start date is August 17, 2009. The closing date for receipt of quotes is July 23, 2009. Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114, no later than 4:30 p.m., on July 23, 2009. The offer must be submitted in a sealed envelope, addressed to this office, showing the solicitation number, and your name and address. Please do not email RFQ's. Qualified vendors will review the following and submit applicable information. STATEMENT OF WORK MEDICAL TECHNOLOGIST SERVICES Qualified contractor shall provide temporary Medical Technologist services for the Pawnee Indian Health Center laboratory, located in Pawnee, Oklahoma. These services will commence on August 17, 2009 to November 13, 2009 for a total of thirteen (13) weeks. Each week will cover forty hours of operation. The contractor shall provide the following services: 1. Collects adequate and correct blood specimens by veni-puncture or micro-technique on adults, children and infants. Follows departmental policies to correctly indentify the patient before any blood specimen is drawn and to correctly label all specimens obtained. 2. Initiates glucose tolerance procedures and collects remaining specimens at required times. Draws other timed specimens, e.g., trough and peak TDM specimens, STAT enzymes, etc. as indicated. 3. Instructs patients on proper collection of samples, e.g. clean catch urine, 24 hour urine collection. 4. Begins instrumentation start-up and shut down procedures. Processes patient samples and conducts analysis as prescribed in laboratory procedure manuals. Performs instrument calibration and required preventive maintenance. Assesses daily quality controls are performed - to establish analysis performance is within predetermined acceptable ranges. Troubleshoots instrument and quality control problems as identified. Maintains section records as required by written protocols. Prepares reagents and verifies instruments are operating correctly. 5. Identifies instrument malfunctions, unacceptable reagent performance, etc., through assessment of quality controls performed. Repeats unusual or unexpected test results and contacts attending physician when "panic value" testing results are found. Initiates appropriate investigative actions when unacceptable analysis, (due to some unknown reason, i.e., instrument dysfunction, quality controls not within acceptable range, etc.) is observed. Using pre-established decision criteria, corrects the observed inadequacies to maintain Quality Assurance. 6. Observes all section safety procedures as applied to contaminated, infectious, or caustic body fluids and/or chemicals. Incumbent must be thoroughly proficient in completing the majority of all section testing procedures available within time limits required by established laboratory protocols. 7. Maintains accurate orderly log records and worksheets where required according to established departmental protocol. 8. At all times displays courtesy - to patient/visitor population, shows concern for and understanding of the patient and promotes the comfort and well-being of the patient as much as possible while performing collection duties. Patience and persistence are needed when dealing with patients from whom blood specimens are difficult to obtain. 9. Projects an image of professionalism in appearance and conduct at all times. INVOICE DOCUMENTS The Contractor shall submit his/her invoice to Financial Management Division, 701 Market Drive, Oklahoma City, Oklahoma, 73114. The Contractor agrees to include the following information on each invoice: (1) Contractor's Name (2) Invoice Date (3) Purchase Order Number (4) Description of services rendered or goods delivered (5) Cost or price (6) Payment Terms (7) Federal Identification Number (EIN) (8) Where practicable, name, title, phone number and complete mailing address of responsible official to whom payment is to be sent. CLAUSES INCORPORATED BY FULL TEXT: Please note that due to character limitation in FedBizOpps, full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulations (HHSAR) can be accessed on the Internet at http://www.farsite.hill.af.mil. The provisions of 52.212-1, Instructions to Offerors-Commercial Items (Nov 2007), applies to this acquisition. In addition to the information required in FAR 52.212-1, Contractors shall provide the following: (1) Dun and Bradstreet Number (note: Contractors must be registered in the Central Contractor Registration to be eligible for award. This can be done at http://www.ccr.gov). The provisions of 52.212-2, Evaluation - Commercial Items (Jan 1999) applies to this acquisition. The Government intends to award one from this solicitation to the responsible offeror(s) whose offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) Price and (2) Qualifications. Both factors are equal in importance. In the event all qualified candidates have the required Qualifications, Price will be the determining factor of award. Quotes shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items (Nov 2007) or indicate certifications in ORCA at https://orca@bpn,gov. D) Certification through American Society for Clinical Pathology (ASCP), American Medical Technologists (AMT), or American Society for Clinical Laboratory Science (ASCLS). E) Two letters of reference from health care providers whom have knowledge of the applicant's skill in Medical Technology.
