- THIS EFFORT HAS BEEN SET ASIDE FOR 8(A) SMALL DISADVANTAGE BUSINESS COMPETITION --- The Centers for Disease Control and Prevention (CDC), Agency for Toxic Substances and Disease Registry (ATSDR) will issue a Request for Proposals (RFP) for a Firm Fixed Price (FFP) contract to develop statewide registry mechanisms that would allow identifying all confirmed cases of Amyotrophic Lateral Sclerosis (ALS) within participating states. Based on ALS Registry Act (H.R. 2295) which was passed on October 16, 2008 amends the Public Health Service Act to requires the Secretary of Department of Health and Human Services (DHHS), acting through the Director of CDC, to (1) develop a system to collect data on amyotrophic lateral sclerosis; and (2) establish a national registry for the collection and storage of ALS data. The CDC anticipates acquiring services to develop statewide registry mechanisms that would allow identifying all confirmed cases of ALS within participating states. The state-based registries would (a) provide reliable data sources to verify and improve completeness of a National ALS Registry; (b) allow for timely population-based incidence and prevalence estimates of ALS in defined geographic areas; (c) provide data for the follow-up for ALS patients, physicians and caregivers to improve the quality of life of people with ALS; (d) facilitate research related to the causes, cures and prognosis of ALS. The successful offeror will utilize a variety of design and implementation strategies to develop reliable and complete statewide ALS registries. It is anticipated that in each state a variety of data sources will be used to identify confirmed cases of ALS. These data sources could include but are no. limited to physician reporting, medical records, death certificates, advocacy group participants, and self-reporting with verification. The anticipated contract period of performance is thirty-three (33) consecutive months, beginning on/or about 09/01/09. Solicitation Number 2009-N-11467 will be posted on the Government wide Point of Entry (GPE) www.fedbizopps.gov on or about July 20, 2009. All responsible sources must submit a proposal in accordance with the released solicitation, which shall be considered by the agency if received on or before the designated due date stated within the solicitation. Interested parties should contact Mr. Michael E. Allen, the CDC Point of Contact for this notice via email: MEAllen@cdc.gov by no-later-than 2:30 PM on July 17, 2009 to confirm their intent to submit an offer following receipt of the CDC's Request for Proposal (RFP).
