- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Purchase Request Number:W81K00-09-T-0231 is issued as a request for quotation (RFQ). This purchase request document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-33. This acquisition is being conducted as a commercial item acquisition in accordance with FAR Part 12. This requirement is 100% Small Business set-aside and all responsible sources may submit a quotation, which will be considered under NAICS code 813110 and the small business size standard is $6.5M. The requirements in this solicitation are: This is a Base plus four option year contract PERIOD OF PERFORMANCE IS Base year 15 July 30 Sept 2009 Option year 1 1-Oct-2009 to 30-Sept-2010 Option year 2 1-Oct-2010 to 30-Sept-2011 Option year 3 1-Oct-2011 to 30-Sept-2012 Option year 4 1-Oct-2012 to 30-Sept-2013 Non-personal service to provide Catholic services for Evans Army Community Hospital, Fort Carson CO in accordance with Statement of Work Attached Statement of Work - SECTION C1 C1 GENERAL REQUIREMENTS C1.1.1 This is a non-personal services contract and all personnel performing under this contract shall remain employees of the contractor and not employees of the Government. All such persons shall be under the control and supervision of the contractor at all times. The contractor shall provide sufficient personnel who meet all requirements herein and who can be relied upon to perform in strict accordance with the contract. The contractor shall provide all documentation of substitute priests and EMEs who are competent, qualified and adequately trained to perform assigned duties to the chief of Department of Ministry and Pastoral Care The contract priest shall be subject to the clinical direction of the Contracting Officer, Contracting Office, Ft. Carson, CO 80913. C1.1 SCOPE OF WORK: The Contractor shall provide one (1) Priest or certified Extraordinary Minister of the Eucharist (EME) from 1 July 2009 thru 30 September 2009 to provide Catholic Mass, visitation, confession and counseling for Evans Army Community Hospital and the Warrior Transition Battalion Catholic population, to include (but not limited to), in-patients, outpatients (as needed/as requested), family members (as needed/ as requested), soldiers and staff. The contactor shall provide weekday Catholic Mass services, Sunday Catholic Mass Worship and sacraments as according to the prescribed Roman Catholic rite, on Ash Wednesday, Holy Thursday, Good Friday, First Fridays of each month and the eight Holy Days. The contractor shall also provide the sacramental services that include: Sacrament of the Eucharist offering of the Sacrament of Reconciliation (confession) and/or administering the sacrament of Anointing of the Sick as needed along with requested counseling. The Roman Catholic Church celebrates seven Sacraments. They are: Eucharist, Baptism, Reconciliation, Marriage, Confirmation, Anointing of the Sick, and Ordination. The contractor will provide for the Sacrament of the Eucharist, Reconciliation (Confession) and Anointing of the Sick (Last Rites) only. All work shall be accomplished within the guidelines of the Roman Catholic Church, Army regulations and directives, and the Installation Chaplains policy statements. The COR and the Department of Ministry and Pastoral care (DMPC), Evans Army Community Hospital (EACH), Fort Carson, Colorado will supervise the contract. All services under this contract are to be performed by an ordained Priest of the Roman Catholic Church or EME and endorsed by the Archdiocese for Military Services, with faculties accepted or granted by the Bishop of the Roman Catholic Diocese of Colorado Springs, CO and in cooperation with the Fort Carson Catholic Community Chaplain. C1.1.2. Performance shall be in accordance with the standards contained in the Performance Work Statement (PWS) and the terms and conditions of Department of Ministry and Pastoral Care Policy Letters. The contract personnel shall abide by EACH Memorandums of Understanding (MOU), policies, rules, regulations and bylaws, as well as applicable Army regulations (ARs) governing such things as medical records, etc. The contract personnel shall perform all services in accordance with the ethical, professional and technical standards defined in the standards of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), Health Care Financing Administration (HCFA) Standards, and the. The contract priest shall be required to comply with regulations governing priest credentialing, privileging and professional performance and demonstrated competence. C1.2 WORK TO BE ACCOMPLISHED will be performed following these specific requirements. C1.2.1 The contractor will report to work at Evans Army Community Hospital, Healer Chapel, on Tuesdays, Sundays and First Fridays. The Contractor will report for work no later than 0900. The contractor is to perform patient visitation, counseling, confessions, other religious rites. Mass is to be preformed at 1200 on weekdays and 1100 on Sunday. C1.2.2 The contractor will perform all services at Evans Army Community Hospital or the Warrior Transition Unit footprint in order for the COR to have adequate oversight of the work being performed. C1.2.3. The COR reserves the right to change or make exception of the location of where the work is to be performed. All exceptions must be discussed with the Chief of Department and Ministry and Pastoral Care. C1.2.4 The contract will report to work at the Warrior Transition Unit Chaplains Office on Thursdays no later than 0900. The contractor is to perform patient visitation, counseling, confessions, and other religious rites. Mass is to be preformed at 1200. C1.2.5 The contractor will be authorized eight (8), hour confession sessions on each of the eight (8) Catholic Holy Days of Obligation. The Holy Days of Obligation are: Mary Mother of God, Epiphany, Ascension of the Lord, The Holy Body & Blood of Christ, Assumption of Mary, All Saints Day, Immaculate Conception, and Christmas. C1.2.6 The contractor will perform no more than 15 hours per week unless authorized by the COR and the Chief of Department of Ministry and Pastoral Care or Evans Army Community Hospital Administrator of the Day (AOD). C1.2.7 The contractor shall be available for national, natural, Mass Casualty, or other emergency deemed by the Evans Army Community Hospital Commander, and will be authorized to remain on Fort Carson to offer 24 hours coverage for a period of no more than 5 days at a cost of no more than $450.00 per day to cover lodging, food and hours worked. C1.2.8 The contractor shall respond to emergency calls form the AOD, which will typically be beyond the 15 hours per week. The contractor will be paid of any emergency situation for an automatic 2 hours of pay at $42.00 per hour. If the contractor is unable to respond he may send a replacement to fulfill the request. C1.2.9 Mass will be preformed at 1200 on Tuesdays and First Friday at Healer Chapel, Evans Army Community Hospital. Mass will be performed at 1200 on Thursday at the Soldier Family Assistance Center, Bldg 1039, Fort Carson, Colorado. Mass will be preformed at 100 on Sundays at Healer Chapel, Evans Army Community Hospital. C1.2.10 The contractor shall provide Mass on the following special occasions: Ash Wednesday, Holy Thursday and Good Friday. The contractor will not be required to visit patients or conduct counseling on the weekday Holy Days of Obligation or weekday special services, only if these days fall on a Monday, Wednesday or Friday other than the First Friday. The contractor will be allowed no more that 3 extra hours of pay on these days if the day does not fall on a Tuesday, Thursday, First Friday or Sunday. C1.3 PERSONNEL. C1.3.1 CONTRACTOR PERSONNEL. All services under this contract are to be preformed by an ordained priest of the Roman Catholic Church or a certified Extraordinary Minister of the Eucharist (EME) and endorsed by the Archdiocese for Military Services. The contractor shall be experienced and fully qualifies to perform the required services without any supervision or direction by Government personnel. C1.3.2 OTHER PERSONNEL. The Contractor shall furnish supervisory, administrative, and direct labor personnel to accomplish all work required. He may not hire off-duty Contracting Officer's Representatives (CORs) or any other person whose employment would result in a conflict of interest under applicable Government laws and regulations. Personnel employed to provide the services described herein shall be competent in the performance of such services. All personnel hired shall abide by EACH MOUs, policies, rules and ARs. All personnel shall maintain and support the standards necessary to meet JCAHO and HCFA certification. C1.3.3 PERSONNEL LIST. The Contractor shall furnish in writing to the COR and/or the Contracting Officers Technical Representative (COTR) promptly upon commencement of work under this contract the complete name, address, social security number (SSN), date of birth and daytime phone number of each primary and alternate priests or EME engaged in performance of this contract. All employees working in place of contractor must meet all requirements listed in contract. In the event of a change in employees, the Contractor shall advise the COR/COTR of the change and shall furnish in writing, within five working days before the change, the same information as stated above. C1.3.4 PERSONAL PROPERTY. The contractor shall ensure that personal property is clearly marked and identified to preclude misidentification as Government property. For example, robes, stoles, communion cup, and/or other ecclesiastical equipment that could be similar to and identified as Government property. C1.4 COVERAGE PLAN. The Catholic Contract Priest must furnish in writing, a Catholic Coverage Plan to the CHIEF DMPC. The Catholic coverage plan should include contact information for Emergency needs of patients after normal duty/work hours. The Catholic Plan should include name/s of the back up Catholic Priest or EME to provide coverage during Catholic Contract Priest absences. The Catholic Coverage Plan is to be provided on 1 August 2009. The Catholic Contract Priest will also submit every two weeks the hours of ministry completed to the Department of Ministry and Pastoral Care Chief and will also be required to submit this time into the DMHRSi system for timekeeping purposes. The Catholic Contract Priest is responsible to personally get the password needed for DMRSi entries (Army MEPRS Program Office guidance). C1.5 EMPLOYEE IDENTIFICATION. The contract priest shall wear an EACH identification badge at all times when in the Hospital. A staff member from the Department of Ministry and Pastoral Care (DMPC) will assist the contract priest in obtaining the identification badge through the EACH S2/S3 Division. C1.5.1 The contractor will obtain a CAC card from the DEERS office on Fort Carson. The CAC card will be used to access computer systems at Evans Army Community Hospital and at the WTU when required. The contractor will be escorted throughout this process by a staff member of the Department of Ministry and Pastoral Care. Once the CAC card is obtained the contractor will register the CAC card with the IMD office at Evans Army Community Hospital in order to obtain an email account. This email account will be used as a form of reliable communication and will be checked on a weekly basis by the contractor. C1.6 EMPLOYEE PERFORMANCE. The Contractor shall not allow any employee to perform work under this contract who is under the influence of alcohol, drugs or any other incapacitating agents. C1.7 CRIME CONTROL ACT OF 1990. The requirements for criminal history background checks will be in accordance with Section J-2, Addendum B, paragraph 2.7. During the credentialing process, the contractor will contact the S2/S3 Division concerning the Crime Control Act of 1990 requirements. C1.8 PERFORMANCE EVALUATION MEETINGS. The Contractor's Project Manager may be required to meet at least weekly with the Contracting Officer or his designee during the first month of the contract. Meetings will be as often as necessary thereafter, as determined by the Contracting Officer or the designee. C1.9 PERFORMANCE EVALUATION The contractor senior evaluator will be the chief of Department of Ministry and Pastoral Care and the Senior Catholic Chaplain of Fort Carson. Together they will submit and annual evaluation report to the Great Plains Regional Contracting office. C1.10 SAFETY. The Contractor shall conduct his operation in strict conformance with all Fort Carson Safety Regulations, Medical Department Activity (MEDDAC) regulations and all applicable provisions of 29 Code of Federal Regulation (CFR) Parts 1910 and 1926 of the Occupational Safety and Health Act (OSHA) (available at the MEDDAC Safety Office). Immediately after a job related injury, the Contractor shall prepare a report of the injury in accordance with the Contractors policy. If an injury is life threatening, the contract priest may utilize the EACH Emergency Room for initial medical treatment. The contract priest will be billed by EACH Treasury Office at the applicable civilian emergency rate. Any technical advice and assistance necessary for reporting and investigating accidents may be requested from the MEDDAC Risk Manager or MEDDAC Safety Manager through the COR/COTR. C1.11 GATE CONTROL. The Fort Carson military installation is a closed post and required post registration of vehicles. Unscheduled gate closures by the Military Police may occur at any time and all personnel entering or exiting the installation may experience a delay in doing so due to inspection of the vehicle, vehicle registrations, wearing of seat belts, etc. The contractor shall not be compensated for delays due to gate closures; however, he will be allowed to make up the time. Response time to Evans shall be within sixty (60) minutes. The Contractor shall furnish his own transportation to and from the work site and will not be reimbursed for travel. The Department of Ministry and Pastoral can will provide a memorandum and a copy of the contract to the Provost Marshall at Gate 1 in order for the contractor to fill out application and obtain Post Tags. C.1.12 QUALIFICATIONS. Each contract priest performing services under this contract shall meet the following licensure, certification, credentialing, education and experience requirements: C1.12.2 The Contractor shall be experienced and fully qualified to perform hospital ministry without any supervision or direction by Government. C1.12.3 Shall ensure that all contract priests providing services under this contract shall be able to read, write, and speak English well enough to effectively communicate with all patients and other health care providers. C1.12.5 Shall be proficient in the areas cited in paragraph C.5.2., Procedures and Tasks.. C1.13 CREDENTIALING REQUIREMENTS: C1.13.1 Should the contractor need to add additional priests during the contract period, the same credentialing process shall apply for each additional individual (See C.5.4.2.). C1.13.2 Contractor shall investigate, verify, maintain and submit documentation of curriculum vitae for each priest proposed to provide services at EACH. Proof of original documentation of ordination and letter of good standing shall be presented and prime source verified. All periods of training must be supported by copies of training certificates. Contractor shall provide documentation of all continuing health education certification. This submission must occur prior to any priest performing work under this contract. The Contractor shall provide the following documentation to permit initiation of a Competency Folder for retention at EACH. Such documentation must show compliance with provisions shown under paragraph C.1.7. and contain the following additional information: C1.13.4 Legible copies of ordination qualifying the provider to perform as a priest, and of any required Clinical pastoral Education (CPE) training certificates involving the intended area of work. All copies must be certified that it is a true copy of the original. C1.13.5 Proof of current competence (within 6 months) and experience in the area of work will be provided in the form of at least two reference letters from the sources listed below; and names, mailing addresses and telephone numbers will be provided to permit direct reference query. Reference letters must be received on letterhead paper directly from the author to EACH (or to the contractor if its a corporation, vice an individual). Letters should address the Catholic Priests professional competence, experience, quality of care, relations with patients and staff members, any personal problems or mental/physical health problems that have interfered with ability to perform the established requirements. C1.13.6 Letter of endorsement from the Archdiocese for Military Services, with faculties accepted or granted by the Bishop of the Roman Catholic Diocese of Colorado Springs, CO and in cooperation with and under the guidance of the Fort Carson Catholic Community Chaplain. C1.13.7 Letter from a peer priest having personal knowledge of the priests professional standing, character and ability. C1.13.8 Statement by a physician regarding physical and mental health, to include any history of substance abuse. Statement of involvement in child abuse cases and claims, including a brief description of the facts of any case settled on behalf of the priest or church, and a history of any disciplinary action taken by church, state boards, or other government agencies. C1.13.9 The Contractor shall submit completed credentials packets for any additional priests 45 calendar days prior to projected start date. Once initial contract is granted, not less than 90 days prior to expiration, EACH will provide renewal documents to the contractor for completion by the priest; completed forms will be returned to the Contracting Officer at least 45 calendar days prior to the expiration of previous privileges. Affiliate privileges are granted for the first year based upon their expiration date and every two years afterwards. C1.13.10 The credentialing documents specified in the above paragraphs will be provided to the Contractor through the Contracting Officer by the NCOIC DMPC in a photocopy-ready state. The Contractor shall reproduce forms as necessary. The Contractor to the Contracting Officer shall submit all completed forms and documentation specified above. C1.13.11 Presentation of a priest to EACH by the Contractor shall, on a continuous basis, constitute evidence that the contractor has investigated and verified all information presented to EACH for the purpose of priest credentialing. During the term of the contract, the contractor shall notify the Contracting Officer within 72 hours upon becoming aware of any information which may compromise a contract priests credentials at EACH. This shall include, but is not limited to, information linking a contract priest with chemical dependence, misdemeanor, or felony. This information shall be reviewed by the EACH Contracting Officer and may result in the rescinding of credentials. The EACH Contracting office is the sole agency authorized to recommend to the Commander that a particular contract priest be granted privileges. The Commander, EACH, is the final authority for approving/denying pastoral privileges to any and all contract priests. C1.13.12 If the credentials of any priest are revoked, he shall not be permitted to continue to provide pastoral care at EACH. Such action will not relieve the Contractor from responsibility to perform the services required by this contract. The contractor shall provide a candidate whose credentials are clearly equal to or grater than those of the original candidate within 30 days after the contractor receives notification that a priests credentials have been revoked. C1.13.13 The contractor shall attend annual/newcomers training at the start of their employment at EACH. The contractor shall complete HIPPA training, CBRNE training and ANTI- Terrorism level 1 during the inprocessing timeframe. The contractor will also ensure that the competency file is maintained. The contractor will be held liable for the information that is needed for the competency files and must provide certificates of training as requested to the staff of the Department of Ministry and Pastoral care. The competency files are to be maintained at Evans Army Community Hospital in compliance with the JCAHO standards. C1.13.14 Incomplete credentials packets will be returned without action. C1.14 HEALTH REQUIREMENTS. The expense for all physical examinations and immunizations required under the provisions of this paragraph shall be borne by the contractor. C1.14.1 All contractor personnel performing direct pastoral care services under this contract shall receive a general physical examination prior to commencement of services. The contractor shall provide the Contracting Officer, 10 calendar days after beginning services under this contract. The certification shall state the date on which the physical examination was completed, the name of the physician who performed the examination, and a statement concerning the physical health of the individual. This certification shall be signed by the physician performing the examination. The requirement for a physical examination may be waived if the physician has had a physical examination within the past year. The contractor is still required to present the certification as stated above along with a statement from the individual direct health care provider stating he has had no significant changes in his/her health since his last physical examination. The physical examination shall include the following: C1.14.2 Test for the antibody to HIV (Human Immunodeficiency Virus) with documented results of the test. C1.14.3 A history to show that direct health care provider has completed a primary series of immunization with tetanus and diphtheria toxins and that a booster dose is current (within the past 10 years). C1.14.4 A test for the hepatitis (Type B) virus with documentation of the results. A profile shall be established to show immune status to hepatitis. Non-immune health care workers (lacking anti-HB(c) or anti-HB(s)) shall be required to complete an immunization series with a Hepatitis-B vaccine (e.g., Recombivax, Engerix). C1.14.5 The physical examination shall also document serologic evidence of immunity to measles and rubella or to provide documentation of immunization with measles, mumps and rubella (MMR) vaccine using the following guidelines: a. Employees born before 1957 without documentation of previous vaccination with MMR should receive one dose. b. Employees born in or after 1957 who have received one dose of MMR previously shall receive one booster dose. c. Employees born in or after 1957 without documentation of any previous vaccination with MMR should receive two doses of vaccine, separated by no less than one month. C1.14.6 Contractor's pastoral care providers shall be screened for tuberculosis by a tuberculin skin test using the Monteux technique. A skin test result of 10 mm of in duration or more shall be required to have a chest roentgenogram and an evaluation performed. A tuberculin skin test of 10 mm of in duration or more will require documentation providing an assessment of the patient (status of infection- active, inactive; need for preventive treatment or not as determined by age, history of BCG (Bacillus Chalmette-Guerin) vaccination; duration of skin test positivity, etc.). If all of the immunizations and tests set forth in the preceding paragraphs have not been completed, the contractor shall issue a certificate providing evidence of immunizations and tests that have been completed or started and shall provide a schedule for the completion of unfinished immunizations and lab tests. After the schedule is completed, the contractor must provide an updated and complete certification. a. All contractor personnel performing direct pastoral care services under this contract, who experience a potential (e.g., needle-stick or cut) or mucous membrane exposure (e.g., splash to the eye or mouth) to blood or bloody body fluids, shall receive prompt treatment. The medical treatment facility (MTF) will evaluate the source of exposure for risk of Hepatitis-B, Hepatitis-C, and HIV and will provide appropriate counseling to the potentially exposed medical provider. The MTF will offer post exposure chemoprophylaxis counseling to prevent HIV. The initial dose is currently recommended within 2 hours after exposure. If necessary, this can be considered an emergency situation and the initial dose provided with follow-up doses given by the priests medical support element. It shall be the contractor's responsibility to provide appropriate treatment as needed to possibly include Tetanus-Diphtheria booster, Immune Globulin, Hepatitis-B vaccine booster, or Hepatitis-B Immune Globulin. The contractor shall be responsible for providing the contract employee with initial testing and if the source of exposure was unknown, positive, or considered at high risk for HIV infection, follow-up testing 3, 6, and 12 months after exposure. In the event of a confirmed or highly suspected potential exposure to HIV, the contractor shall insure that the contract provider receives appropriate counseling and is referred immediately to a private infectious disease specialist for consideration of any experimental therapy (e.g., AZT). The Government may require the contractor to provide evidence of the status of treatment and testing of the individual provider under the contract. b. All contract direct pastoral care providers shall receive the current influenza immunization by 1 January of each year unless contraindicated by allergy to eggs. The contractor shall provide the Contracting Officer a certification 10 calendar days after immunization is completed. The certification shall state the date on which the influenza immunization was completed and the name of the physician who provided the immunization. c. Failure to meet the requirements stated herein, or when test results determine a contract priest has a contagious disease, the contracting officer may, upon the advice of the MTF Commander or his clinical staff, determine that such priest is not an acceptable individual to perform services under this contract. C1.14.7 Backup personnel shall be required to provide equally current certification of health at the time of initial request for pastoral privileges, and annually thereafter. The expense for all physical examinations required under the provisions of this paragraph shall be borne by the contractor. C1.15 OTHER REQUIREMENTS. C1.15.1 The contractor is responsible for providing priests who are adequately rested and fully physically and mentally capable of performing the duties required under this contract. C1.15.2 The contractor will fulfill all required sacramental services that are required under this contract and per the designated schedule. If unable to do so, it shall be the contractors obligation to find another replacement priest to fulfill the requirements of the sacraments. The contractor will also be obligated to find a Extraordinary Minister of the Eucharist (EME) who is authorized by the Archdiocese for the Military services, a registered volunteer, HIPPA trained and able to receive a hospital identification badge. The contractor is then obligated to pay the replacements at the same hourly rate. C1.15.3 The Catholic Contract Priest shall receive a lab coat wear during visitation of in-patients. The Lab Coat will clearly designate Catholic Priest with a name plate or embroidery so the patients do not confuse the Catholic Priest with a Doctor. The Catholic Priest may opt to wear his clerics instead of the lab coat. C1.15.4 The contract priests must conduct themselves in a professional manner at all times. Validated patient, staff or parishioner complaints may result in removal of the contract priest. The contractor shall provide a suitable qualified replacement within 10 workdays after the contractor receives notification that a priest will be removed. C1.16 TOBACCO CONTROL. The Contractor and his employee's shall observe the EACH Smoking/Tobacco Policy. C1.17 COMPUTER TRAINING: The Catholic Contract Priest will be provided training from DMPC on the CHCS system for acquiring the Catholic Patient List in the event that the Catholic Patient List does not come through the normal system when needed by the Catholic Contract Priest. CHCS training will be provided at no cost to the Catholic Contract Priest. He will also be provided training in electronic charting, so the contractor may chart in the patients records after visitation. C1.17.1 Composite Healthcare Computer System (CHCS). CHCS contains the MTFs appointment scheduling program, and is used to identify a patients religious preference for visitation and is interlinked with other departments in the MTF. C1.17.2 Training for CHCS will be provided to the contractor as soon as possible. The training will be conducted by IMD personnel and will be on-site and during normal duty hours. This training will be at no cost to the contractor. This training is to be completed within the first 60 days from when the contract is granted. C1.17.3 If additional training is needed due to mission changes, the MTF Information Management Office will conduct required computer training for contract priests who need access to the systems used in the MTF to be able to perform under the terms of this contract. This training will be at no cost to the contractor. PAD personnel will schedule this training. The MTF will not reimburse the contractor for any expenses incurred while undergoing any computer training. SECTION C2 C2 DEFINITIONS. The following are standard Government terms: C2.1 ADVISORY DOCUMENT. Those directives which the Contractor may use for information and guidance but are not binding for compliance. C2.2 BENEFICIARIES OF THE MILITARY HEALTHCARE SYSTEM. Those individuals entitled to care at the MTF in accordance with AR 40-3. C2.3 COMPOSITE HEALTHCARE COMPUTER SYSTEM (CHCS). Identifies a computer system that contains the MTFs appointment scheduling program, pharmacy/lab/radiology ordering system and is interlinked with other departments in the MTF. C2.4 CONTRACT SPECIALIST. The official Government representative delegated authority by the Contracting Officer to administer a contract. This individual, normally working in an appropriate Contracting/Procurement career field, advises on all technical contractual matters. C2.5 CONTRACTING OFFICER (CO/KO). A person who either by virtue of his/her position and appointment has authority to enter into and administer contracts and make determinations and findings with respect thereto. The Contracting Officer is the only person with authority to make changes to the contract and bind the Government. C2.6 CONTRACTING OFFICER'S REPRESENTATIVE (COR). An individual designated in writing by the Contracting Officer to perform technical surveillance as specified in appointment letter. C2.7 INSPECTIONS OF SERVICES CLAUSE SUMMARY (ISCS). Identifies selected service outputs of the contract that will be evaluated by the Government, in accordance with the Inspection of Services Clause, to assure contract performance standards are met by the contractor. C2.8 MANDATORY DOCUMENTS. The method specified in the directives shall be performed by the Contractor to meet the stated results of the directives. C2.9. OUTCOMES MANAGEMENT. Multi-disciplinary health care delivery processes whose goals are to provide quality health care, decrease fragmentation, enhance patient care outcomes and constrain costs. C2.10 QUALITY ASSURANCE. A methodology used by the Government Contracting Officer to assure that the quality of purchased goods and services received are acceptable and in accordance with established standards and requirements of this contract. C2.11 QUALITY CONTROL. A methodology used by the Contractor to control the quality of goods and/or services provided. C2.12 PERFORMANCE IMPROVEMENT: A systematic methodology used by EACH to assess and improve healthcare processes and outcomes in an ongoing and continuous fashion. It includes elements of quality control and quality assurance, as well as specific management tools and techniques to achieve measurable improvement in key hospital functions and significant aspects of healthcare performance. C2.13 RISK MANAGEMENT. An element of quality assurance used by EACH which assesses and strives to reduce the medico legal risk associated with individual cases, patterns of care, or hospital systems. C2.14 UTILIZATION MANAGEMENT. An element of performance improvement used by EACH which assesses and strives to improve the effective use of hospital resources, including personnel, supplies, equipment and finances. C2.15 ACRONYMS. The following acronyms are used in this PWS. GOVERNMENT FURNISHED PROPERTY, FACILITIES AND SERVICES. C3.1. GENERAL. The Government will provide the following: C3.1.1 Shared uncluttered office working space that will include a desk, chair, lights and office supplies. This space will not be used for counseling or confessions. The Contractor shall not make any structural changes without prior approval of the COR. Structural repairs required during the term of the contract shall be reported to the COR for appropriate action. The Contractor shall reimburse the Government for repairs not attributable to fair wear and tear. The government will provide adequate neat, uncluttered storage space for Catholic literature and material. The storage space will be lockable in order to store sensitive material. One key will be provided to the contractor and the Department of Ministry and Pastoral Care will also maintain a key. C3.1.2 The Government will provide all structural building maintenance except as specified in paragraph C.3.1.1. above and housekeeping services at no cost to the contractor. C3.1.3 The priests performing under this contact may use the EACH dining facility and pay the normal rate for food. C3.1.4 Adequate neat uncluttered work space in the storage/working room will be provided to store Catholic items that will be made available to the Catholic Priest to be used in confession, patient visitation and confession. A storage locker will be provided that will hold all of the Catholic ecclesiastical supplies. C3.1.5 The government will furnish at the above mentioned locations with equipment for the support of the contractor only in the actual performance of services. C3.1.6 The government will provide all required liturgical items of equipment and supplies needed in the conduct the required services. The cost for these items has to be approved by the Department of Ministry and Pastoral Care. The requested items must be presented to the Chief of Department on Ministry and Pastoral Care and must be approved before the items are ordered. The contractor will then receive a copy of the request for the order. C3.2 Three week notification of special needs in order to effectively meet the needs of this contract have to be in writing and given to the Chief of the Department of Ministry and Pastoral Care. The ordering time will vary due to the time it takes to process order through the logistics department at Evans Army Community Hospital and the process of requesting purchase orders if deemed necessary. C3.3 The Post Catholic Bulletin and the Local Catholic Newspaper will be made available to the Catholic population at Evans Army Community Hospital as long it remains accessible to the Department of Ministry and Pastoral Care. C3.4 Monthly dialogue and feedback meeting with the Chief of Department of Ministry and Pastoral Care and staff members is available. C3.5 Provide the protection of the Blessed Holy Sacrament from unauthorized and inappropriate use. C3.6 Adequate supply, resupply, and the purchase of new items or refurbishing of repairable equipment as needed. C3.7 The contractor may use personal ecclesiastical and liturgical items of equipment to conduct the Sacraments that are required within this contract. The Government is not liable for loss, damage, theft or maintenance of personal ecclesiastical equipment. C3.8 TELEPHONE SERVICES. The Government will furnish Class A (on and off post and long distance calling capabilities) telephone service. C3.9 PUBLICATIONS AND FORMS. The Government will make available to the Contractor all required forms and publications listed at Section C6. SECTION C4 SPECIFIC TASKS C4 PERFORMANCE WORK STATEMENT. C4.1 The contractor will report to work at Evans Army Community Hospital, Healer Chapel, on Tuesdays, Sundays and First Fridays. The Contractor will report for work no later than 0900. The contractor is to perform Weekday Mass, Sunday Mass, patient visitation, counseling, confessions, other religious rites. Mass is to be preformed at 1200 on weekdays and 1100 on Sunday C4.1.2 The contractor will perform all services at Evans Army Community Hospital or the Warrior Transition Unit footprint in order for the COR to have adequate oversight of the work being performed. C4.1.3 The COR reserves the right to change or make exception of the location of where the work is to be performed. All exceptions must be discussed with the Chief of Department and Ministry and Pastoral Care. C4.1.4 The contract will report to work at the Warrior Transition Unit Chaplains Office on Thursdays no later than 0900. The contractor is to perform Catholic Mass, patient visitation, counseling, confessions, and other religious rites. Catholic Mass is to be preformed at 1200. C4.1.5 The contractor will be authorized eight (8), hour confession sessions on each of the eight (8) Catholic Holy Days of Obligation. The Holy Days of Obligation are: Mary Mother of God, Epiphany, Ascension of the Lord, The Holy Body & Blood of Christ, Assumption of Mary, All Saints Day, Immaculate Conception, and Christmas. C4.1.6. The contractor will perform no more than 15 hours per week for counseling, visitation and confession, unless authorized by the COR and the Chief of Department of Ministry and Pastoral Care or Evans Army Community Hospital Administrator of the Day (AOD). C4.1.7 The contractor shall be available for national, natural, Mass Casualty, or other emergency deemed by the Evans Army Community Hospital Commander, and will be authorized to remain on Fort Carson to offer 24 hours coverage for a period of no more than 5 days at a cost of no more than $450.00 per day to cover lodging, food and hours worked. C4.1.8 The contractor shall respond to emergency calls form the AOD, which will typically be beyond the 15 hours per week. The contractor will be paid of any emergency situation for an automatic 2 hours of pay at $42.00 per hour. If the contractor is unable to respond he may send a replacement to fulfill the request. The Chief of DMPC can require any additional days that are not mentioned above, if a special occasion is needed. C4.1.9 Initial patient visit 30 Minutes C4.1.10 Average patient follow-up visit 15 minutes C4.1.11 Mass will be preformed at 1200 on Tuesdays and First Friday at Healer Chapel, Evans Army Community Hospital. Mass will be performed at 1200 on Thursday at the Soldier Family Assistance Center, Bldg 1039, Fort Carson, Colorado. Mass will be preformed at 100 on Sundays at Healer Chapel, Evans Army Community Hospital. C4.1.12 The contractor shall provide Mass on the following special occasions: Ash Wednesday, Holy Thursday and Good Friday. The contractor will not be required to visit patients or conduct counseling on the weekday Holy Days of Obligation or weekday special services, only if these days fall on a Monday, Wednesday or Friday other than the First Friday. The contractor will be allowed no more that 3 extra hours of pay on these days if the day does not fall on a Tuesday, Thursday, First Friday or Sunday. C4.2. ABSENCES. The contract priest will be authorized a maximum of fifteen (15) working days, or a total of 60 hours, of absences, for sick or vacation purposes. If the contract period is less than 12 months, the authorized working days/hours of absences per FTE will be prorated. Absences will not be compensated by the Government. All requests for vacation must be submitted in writing a minimum of 30 calendar days in advance to the Chief of DMPC. The amount of scheduled excused absences used at any one time shall be at the discretion of the Chief of Department of Ministry and Pastoral Care and the needs of the Warrior Transition Battalion. The contractor shall notify the Chief of Department of Ministry and Pastoral Care within one (1) hour of scheduled reporting time, if he is sick and cannot report to work as scheduled. If an emergency absence should arise the contractor shall notify the Chief of Department of Ministry and Pastoral care as soon as physically possible. The contractor has the option to provide backup coverage for contract to qualified Priests to cover all absences. Backup contract Priest will have to meet the same training, certification, licensure and privileging requirements as the primary contract Priest. All use of backup Priests shall be coordinated with and preapproved by the Chief of Department of Ministry and Pastoral Care. The authorized absences specified in this paragraph are applicable to the primary Contract Priest only and do not apply to the backup Priests. C4.2.1 The government reserves the right to verify the hours worked by the contract Priest by implementing sign-in/sign-out procedures C4.3 The contractor shall not employ persons convicted of violations of criminal statutes, misdemeanors, or felonies involving moral turpitude. The government reserves the right to require removal from the job site any contract employee who endangers persons or property, whose continued employment is inconsistent with the interest of military security or who is found to be incapacitated or under the influence of alcohol, drugs or other substances. Removal of employees for any reason does not relieve the Contractor of the requirement to perform services specified herein. The contractor shall provide a candidate whose credentials are clearly equal to or greater than those of the original candidate within 10 days after contractor receives notification that Priest will no longer be available or is no longer available. C4.3.1 The contractor shall not bill a patient, an insurer, or anyone else for services. The only compensation that the contractor is entitled to for performance of the contract is payment as the contract specifies. This includes donations that the contractor may receive for performing a requested Sacrament. C4.3.2 The contractor is not prohibited by reason of their employment, under this contract, from conducting a private parish, so long as there is no conflict with the performance of services under this contract. C4.3.3 The contractor shall make no use of any Government facilities or other Government property in connection with conducting a private parish. C4.3.4 The contractor or contract physician shall, without additional change in contract price, be responsible for obtaining any necessary documentations and certificates. C4.4 ADMINISTRATIVE PROCEDURES. The contract priest is required to generate and maintain proper records on beneficiaries to whom treatment is provided in accordance with AR 40-66, Medical Record Administration. All records generated in the performance of the contract will remain the property of and subject to exclusive control of the government. C4.5 CONTINUITY OF SERVICES: The contractor shall provide full coverage for the hours of performance shown in paragraph C.4.1 The contractor shall not use temporary, short term Priests on a continuous basis in the performance of this contract. C4.5.1 All contractor backup Priest proposed to cover during absences of the primary Priest will have to meet the same training, certification, licensure and privileged requirements as the primary Priest Documentation on backup Priest shall be submitted, in writing, to the Chief of Department of Ministry and Pastoral Care for approval. Credentials must be submitted no later than 30 calendar days prior to the backup Priest performing under this contract. C4.5.2 The Chief of Department of Ministry and Pastoral Care, shall notify the Contractor immediately when staffing changes develop which affect the scheduling of any service. C4.6 PERFORMANCE IMPROVEMENT/UTILIZATION REVIEW/ RISK MANAGEMENT: All Joint Commission of Accreditation of Healthcare Organizations (JCAHO) Standards shall be met and maintained during the performance of this contract. C4.6.1 Continuing review and investigation of all complaints against contract Priest will be recorded and submitted to the Great Plains Regional Medical Command Contracting Office. C4.7 GOVERNMENT QUALITY ASSESSMENT AND IMPROVEMENT (QA&I). The Government will monitor the contractor's performance by utilizing scheduled and unscheduled reviews by of Installation Command Chaplains Office and the Chief of the Department of Ministry and Pastoral Care. C4.8 CONTRACT SURVEILLANCE. The government reserves the right to have designated MTF personnel (e.g. COR/COTR) enter government-supplied facilities furnished for the purposes of contract personnel to perform under this agreement at any time for surveillance purposes as required. C4.9 Shall be an ordained Priest of the Roman Catholic Church and endorsed by the Archdiocese for Military Services, with faculties accepted or granted by the Bishop of the Roman Catholic Diocese of Colorado Springs, CO and in cooperation with and under the guidance of the Fort Carson Catholic Community Chaplain.. C4.10 Shall possess sufficient initiative, interpersonal relationship skills and social sensitivity such that he can relate constructively to a variety of patients from diverse backgrounds.
