Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 12.6, with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32 as corrected. This solicitation is unrestricted. The NAICS code is 621111, with a size standard of $10.0 million. The delivery, acceptance, and FOB point is the VASNHCS in Reno, NV. The contract will include a six-month period from August 01, 2009 to July 31, 2010. Only electronic offers will be accepted. Offers are due to joel.burgess@va.gov by NLT 3:00 p.m. on July 17, 2009. Scope of Work: The contactor shall provide board certified or board eligible physicians within the specialty of Cardiology to the VA Sierra Nevada Healthcare System (VASNHCS). The services required will be a full range of cardiology services for outpatient VA beneficiaries including consultation, diagnosis, therapy and treatment, and prognosis of patients with cardiology disorders. Services will be performed at VASNHCS. Services will include a minimum of eight half-day outpatient clinics per week equal to 80% of one full time equivalent (.8 FTE) employee. 2. Specific Contractor tasks include: 2.1. The Contractor shall provide a minimum of eight half day cardiology clinics at VASNHCS each week. Patients will be scheduled by VASNHCS. The clinics will include stress testing, reading and interpretation of echocardiograms, Holters, and electrocardiograms. 2.1.1 The Contractor will be responsible for interpretation and reporting of approximately 250 echocardiograms per year. 2.1.2. The Contractor will be responsible for interpretation and reporting of approximately 208 Holter tests per year. 2.1.3. The Contractor will be responsible for interpretation and reporting of approximately 2,850 electrocardiograms per year. 2.1.4 The day and time of each clinic will be set a minimum of 45 days prior to the scheduled clinic to allow time for scheduling patients. The dates and times for clinics will be approved by the Chief, Medical Service or designee after reviewing the Contractor's recommended schedule submission. The Contractor's schedule shall include the date, time, and assigned provider for each clinic. A full explanation will be provided to the Contractor for any date or time that has been disapproved. An alternate date and time will be mutually agreed upon. 2.15. In the event that a procedure clinic is cancelled for any reason, the Contracting Officer's Technical Representative (COTR) or designee shall be notified immediately and the clinic will be rescheduled after a new date and time has been approved. The Contractor shall be responsible for contacting each patient to reschedule their appointments for the new procedure clinic when cancellations occur less than 45 days of the scheduled procedure clinic. In the event the contractor cannot reschedule a clinic the VA reserves the right to reduce the amount equivalent to the reduced service. 2.2. Normal Work Hours. The normal work hours for VASNHCS are 8:00 am to 4:00 pm Monday through Friday. All on-station clinics will be scheduled in four hour increments twice per week within normal work hours. See Paragraph 2.1.4. above regarding scheduling. 2.3. Standards: Comply with all applicable VA guidelines; medical center medical staff by laws, rules and regulations; hospital polices/procedures; and in accordance with current Joint Commission standards. The Contractor shall document evaluation and treatment recommendations in the VA electronic medical record in accordance with VASNHCS Directive MOIC-003-08 (Attachment 3). All progress notes, consults, discharge summaries, and/or clinical and surgical procedure reports, etc. must be completed (including signature) within 24 hours. See Section II, Paragraphs 5, 6, and 7 below regarding Medical Records. 2.4. Quality: Contractor physicians shall participate in facility Quality Improvement and Safety Programs, as well as peer reviews when required by the Chief, Medical Service or designated representative. Additionally the contractor physician shall participate in continuous performance improvement activities as required. 2.4.1. To enhance patient care and satisfaction, performance measures will be established by VASNHCS or implemented as mandated by the Veterans Health Administration (VHA) and/or the Sierra Pacific Network (VISN 21) as goals. The Contractor shall adjust their work effort to meet these goals. 2.4.2. Performance measures include VA interest items such as timely appointments for new and follow-up patients, quantity of patients seen per day, clinical practice guidelines, preventive medicine, and Undersecretary monitors. Performance measures and monitors may change as patient needs arise. The contractor will be provided with the table of measures and monitors annually or as changes occur. 2.4.3. Participate in regularly scheduled meetings to discuss improvements to meet performance measures and other quality assurance issues when standards are not met. Meetings will be scheduled at the discretion of the Chief, Medical Service during on-station clinics. 3. Normal Duty Hours. VA normal duty hours are Monday through Friday, 8:00 am through 5:00 pm. The Contractor will work during non-duty hours as follows: "On-Call weeknights shall be Monday through Friday 5:00 pm to 8:00 am. "On-Call weekend and holidays shall be Saturday, Sunday, and Holidays from 8:00 am to the following day at 8:00 am. See Section A of the Contract for VA recognized holidays. "Rounds shall be conducted between 8:00 am and 4:30 pm on weekends and holidays depending on the complexity of the patient population. On occasion, rounds will include liaison and consultation assessments in the Medical and Surgical wards. "Call backs may be required when clinically indicated. The Contractor will be paid at the rate stipulated in the Price Schedule for call back requirements. 4. Invoicing: 4.1. On-Station Invoicing: At the end of each month, the Contractor shall provide the COTR a spread-sheet that includes the name of each patient, full social security number, dates treated, and diagnostic and/or procedural codes. The price for on-station services will be based on the unit price for an endoscopy procedure clinic at the rate stipulated in the contract price schedule. 4.2 Invoices must be forwarded by the contractor electronically directly to the COTR for Medical Service E-mail address will be provided to the Contractor by the COTR after award of the contract. 5. Qualifications. 5.1 The Contractor will be responsible for ensuring that each physician providing services under this contract is fully trained and completely competent to perform the required services covered by this contract. Contractor physicians shall be at a minimum: 5.2 Licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia, 5.3 Board Certified or eligible in the practice of Cardiology, 5.4 Hold a current, unrestricted Medical License, 5.5 Must be registered in VETPRO, and 5.6. Must meet medical staff criteria for initial appointment and reappraisal in accordance with VHA Handbook 1100.19, entitled Credentialing and Privileging (see Attachment 2) to practice medicine at VASNHCS prior to beginning work.
