Evaluation Criteria and Basis for Award Statement of Work for Base & 2 Option Years Instructions for submitting Proposals & Questions Regarding the Solicitation Option Year #2 Proposal Sheet Option Year #1 Proposal Sheet Base Year Proposal The U.S. Department of Labor, MSHA (Mine Safety & Health Administration) is seeking Firm Fixed Price proposals for administrative contract employees located in Beaver, WV and Wilkes-Barre, PA. The National Mine Health & Safety Academy is located in Raleigh County, at 1301 Airport Road, Beaver, WV 25813 and the MSHA Assessment Center is in Luzerne County, at 7 N. Wilkes-Barre Boulevard, Suite 432, Wilkes-Barre, PA 18702. We currently have 10 (ten) contract employees at the Beaver, WV location and 1 (one) at the Wilkes-Barre, PA location. Contractor must be able to provide service at both locations. Proposals submitted for only one location will not be evaluated. Please provide Firm Fixed Price proposal for the 10 (ten) contract employees at the Beaver, WV location based on Wage Determination No. 2005-2573, Revision No. 7, Dated 05/26/2009, plus any fringe benefits you may offer. All terms and conditions of the Service Contract Act apply (i.e. health & welfare benefits).* Please indicate how benefits package is administered. Please provide Firm Fixed Price proposal for the 1 (one) contract employee at the Wilkes-Barre, PA location based on Wage Determination No. 2005-2453, Revision No. 7, Dated 05/26/2009, plus any fringe benefits you may offer. All terms and conditions of the Service Contract Act apply (i.e. health & welfare benefits).* Please indicate how benefits package is administered. *Pricing subject to change at beginning of each Option Year based on current Wage Determination per location. Period of Performance will be from award date of this contract and continuing for 12 (twelve) months and 2 (two) option years (Attachments #1, #2, #3). Proposals must include Firm-Fixed pricing for a base and 2 (two) option year periods. Please note that the invoicing rate should be based on 2080 hours. The 2080 hours is an estimate of the annual hours required and does not guarantee a minimum level of performance. All option years are subject to availability of funding. Proposals that do not include base and all option years will not be considered. NOTE: Proposals shall not be sent via e-mail. Offerors are required to provide a technical and price proposal and 3 (three) past performance references for contracts held that are similar to the work to be performed. The successful offeror will be required to submit a written offer signed by a corporate officer with authority to commit the company to performance. Award of this contract will be based on Best Value for the Government, pricing and other factors considered. A description of duties is attached for all 11 (eleven) positions (Attachment #4). Offerors must comply with Executive Order #13495 dated January 30, 2009, regarding displaced qualified workers under service contracts. THIS ACQUISITION IS A SMALL, SERVICE DISABLED VETERAN OWNED BUSINESS SET-ASIDE.
