- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W90GUX91242000 is issued as a Request for Quote (RFQ). This request is due by 17 June 2009, 12:00pm (Central Time). The RFQ will result in a firm fixed price contract. This solicitation is 100% set aside for small business. The North American Industry Classification System (NAICS) code for this acquistion is 624410. This requirement is for a non-personal service. Yellow Ribbon Family Services during Deployment Mississippi Army National Guard Performance Based Work Statement 1. Scope of Work: 1.1. This is a non-personal service contract. The contractor shall provide qualified personnel to provide child care to the Mississippi Army National Guard Yellow Ribbon Family Services during pre-deployment events to be held 2days (Saturdays & Sundays) for approximately 16 hours total for the weekend. 1.2. All providers must be licensed or certified by the State of Mississippi and have the appropriate number of certified attendees available for each event. 1.3. Hours of Performance: Childcare shall be provided on designated days, 7:00am to 4:00pm. 1.4. The government reserves the right to verify the hours worked by the Contractor by implementing sign-in/sign-out procedures or any other means deemed necessary by the Contracting Officers Representative (COR). 1.5. Personal Appearance: Contractor personnel working under this contract shall practice high standards of personal hygiene and maintain a clean, neat appearance while on duty Clothing shall be clean and neatly pressed; free from lint, debris, dirt, mud, stains, or other substances; and void of rips, tears, and holes. 1.6 Contractor personnel shall be free of any contagious diseases. 2. Definitions 2.1. CONTRACTING OFFICER: A person duly appointed with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings on behalf of the government. 2.2. CONTRACTING OFFICERS REPRESENTATIVE: (COR) a government employee selected and designated in writing by the contracting officer to act as his/her designated representative in administering a contract. 2.3. QUALITY CONTROL: Those actions taken by the contractor to control the services provided to ensure they meet the requirements of the PBWS. 2.4. QUALITY ASSESSMENT AND IMPROVEMENT: Those actions taken by the government to check services to determine that they meet the requirements of the PBWS. 3. Specific Tasks 3.1. The contractor shall provide child care to the Mississippi Army National Guard Yellow Ribbon Family Services during deployment for 25 children (0-4YR) for 16hours. 3.2. All personnel, equipment, supplies and materials necessary to furnish a full time quality day care. Provider must supply all equipment required to care for 0-4 age group: Playpens, baby beds, nap pads, toys, etc. Must also provide disposable diapers, wipes, pull ups, lotions, powders and other appropriate toiletry items. 3.3. Providers to provide low sugar snack items and juices appropriate for age group (children have meals with parents who are attending meeting in same building or complex). 3.4. Providers must be mobile and be available from 7:00AM 4:00PM for programs scheduled for Saturday and/or Sunday events scheduled in various cities in the State of Mississippi. Below are detailed services required on specific dates and cities: July 11, 2009 Desoto County Civic Center July 12, 2009 Jackson, MS July 18, 2009 Cleveland, MS July 19, 2009 Brookhaven, MS Aug 1-2, 2009 Gulfport, MS Aug 8, 2009 Gulfport, MS Aug 9, 2009 Hattiesburg, MS Aug 15, 2009 Tupelo, MS Aug 16, 2009 Meridian, MS Sept 12&13, 2009 Jackson, MS 3.5. Providers are responsible for any/all travel expenses (overnight rooms, meals, etc.) incurred. 3.6. Contractor shall ensure ratio of caregiver to children. Number of providers (1/6 ratio) determined by number of children, ages 0 4 a determined by pre-registration application received. 3.7. The contractor shall provide an invoice to the COR for all hours and partial hours care was rendered, within five (5) calendar days of the end of the month of service The invoice shall include a signed copy of page 1 of the SF 1449. 3.8. Children must be continually accounted for. 4. Applicable Regulations and Manuals 4.1. Regulatory documents pertinent to this performance work statement are listed below. Offeror must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov. 4.3. Provider must provide Federal Tax ID#, NCAIS #, DUN & BRADSTREET # and CCR#. 4.4. Should any supplement, amendment or change cause a change in the scope of performance within the meaning of the Changes clause, it will not be implemented by the contractor unless or until a modification is issued by the contracting officer.
