Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-32. The small business size standard is $7 million average annual receipts for the past three years. This procurement is unrestricted. Offerors are encouraged to submit quotes based on the work requirements specified herein for a base period of July 1, 2009 through September 30, 2009, with four pre-priced renewal option years from October 1, 2009 through September 30, 2013. Proposed rates should include all costs associated with Request For Proposal (RFP) VA-251-09-RP-0173 and should include the following line items for base period and each renewal option year: Item 101. Seven trips from VA Outpatient Clinic, Grand Rapids, MI to VA Medical Center, Battle Creek, MI (Estimate 65 miles one way). Item102. Five trips from, VA Outpatient Clinic, Grand Rapids, MI to VA Medical Center, Ann Arbor, MI (Estimate 131 miles One Way). Item 103. One trip From VA Outpatient Clinic, Grand Rapids, MI to VA Medical Center, Saginaw, MI (Estimate 115 miles One Way). Item 104. One trip From VA Outpatient Clinic, Grand Rapids, MI to VA Medical Center, Detroit, MI (Estimate 215 miles One Way). Item 105. Ten trips From VA Outpatient Clinic, Grand Rapids, MI to within the Grand Rapids, MI city limits. Item 106. Five trips with attendant (when required and ordered). STATEMENT OF WORK - Taxi Service GROPC - C.0. SCOPE: Provide taxi cab services, with or without an attendant for the Department of Veterans Affairs (VA) Outpatient Clinic, Grand Rapids, Michigan between the hours of 8:00 am to 5:00 pm departure time, Monday thru Friday, excluding holidays. Transportation services required shall be within the city limits of Grand Rapids and are primarily to the VA Medical Center, 5500 Armstrong Road, Battle Creek, Michigan, but may include transportation to other VA facilities located in Ann Arbor, Detroit, and Saginaw, Michigan. C.1.0. Qualifications: C.1.1. Quotes will be considered only from those offerors who are established in the business of Taxi Service and who are financially responsible and have the necessary equipment and personnel to furnish services in the volume required for all the items under this contract. Successful offeror shall meet all requirements of Federal, State or City codes regarding operations of this type of service. C.1.2. each offeror shall submit, with the quote, a letter in duplicate fully describing the make of vehicles, model and year which he/she agrees to furnish under this proposal including the locations and telephone numbers of his/her establishment where calls are received and vehicles are immediately available for dispatch. C.1.3. The right is reserved to thoroughly inspect and investigate the establishment, facilities, business reputation and other qualifications of any offer or and to reject any quote, irrespective of price, that shall be administratively determined lacking in any of the essentials necessary to assure acceptable standards of performance. C.2.0. DRIVERS/ATTENDANTS: C.2.1. A record of each employee as to character and physical capabilities of performing the duties of a taxi driver or attendant shall be maintained and made available for inspection upon request. C.2.2. All drivers and attendants shall have attained legal age (the age of 21 years) and shall maintain neat, clean and professional appearance. Drivers shall be licensed in accordance with the State of Michigan. Attendants shall be able to communicate with patients in English using tact and courtesy. C.3.0. VEHICLES: C.3.1. Vehicles used in the performance of this contract shall be licensed and meet all minimum vehicle requirements as mandated by the State of Michigan. C.4.0. ORDERING: C.4.1. Requests for services will be made by telephone. The contractor shall be required to respond to all requests within thirty (30) minutes. If a vehicle does not respond, the VA reserves the right to obtain the service from another source, and to charge the contractor with any excess cost which may result therefrom. The contractor shall be required to provide a minimum of two (2) vehicles at any one time; however, should the VA order additional vehicles simultaneously and the contractor is able to provide additional vehicles, vehicles will be provided at contract cost. VA will be the sole judge in determining when to order service from another source. However, in no instance will the contractor be required to provide more than three (3) vehicles at one time. C.5.0. NUMBER OF PATIENTS: C.5.1. It is understood and agreed that only ONE beneficiary be transported on a trip unless specifically authorized by the Department of Veterans Affairs. When more than one beneficiary is transported on a trip, reimbursement will be made at a rate not to exceed the cost of transporting a single beneficiary. C.6.0. LIMITATIONS: C.6.1. Under no circumstances shall any person other than the driver, attendant (if ordered), and the beneficiary ride in a vehicle, except as requested by the therapist. Exception may include the beneficiaries wife, significant other, or the police. C.7.0. SECURITY: C.7.1. While on VA grounds, contractor must secure vehicles when left unattended. If vehicle must be left unattended with motor running, a second set of keys will be necessary, so that vehicle can be securely locked. The provision at 52.212-1, Instructions to Offerors Commercial (Jun 2008), applies to this acquisition. The provision at 52.212-2, Evaluation - Commercial Items (JAN 1999), applies to this procurement. Offerors are required to submit the following documentation with proposals: Offerors are required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, (Feb 2009). Offerors must submit proposals on the attached pricing schedule. The awarded contractor will be required to submit a signed SF1449 prior to award.
