- This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Request for Quotation (RFQ) number is VA-246-09-RP-0127. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2000-07. This is 100% set-aside for small business. NAICS code is 811219. Size standard is $7.0 Million. The VA Medical Center, Durham, North Carolina intends to award a firm fixed price contract for Preventative Maintenance on ONI Model Extremity, MRI (1.OT). The service includes coverage for 145mm, 100mm, and 80mm Imaging Coils. Repairs must be accomplished by an authorized service representative. Contractor shall provide full services to include all parts, labor, travel, software updates and quarterly preventative maintenances services necessary to maintain this equipment. Contract shall be awarded for one Base Year plus 4 Option Years. FAR 52.212-1 Instructions to Offerors--Commercial Items applies. Evaluation will be in accordance with FAR 52.212-2 Evaluation--Commercial Items. Evaluation factors are technical capability, price and past performance. Each is equally important. Offerors must include a completed copy of the provision at FAR 52.212-3, Offeror Representatiions and Certifications - Commercial Items, with its offer. FAR 52.212-4, Contract Terms and Conditions--Commercial Items and 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items applies.
