- Louis Stokes Cleveland VAMCDepartment of Veterans Affairs VISN 10 Contracting Office 10000 Brecksville Road Brecksville, Ohio 44141-3204 JUSTIFICATION FOR OTHER THAN FULL AND OPEN COMPETITION May 27, 2009 IFCAP Transaction #: 541-09-3-5756-0633 Identification of Agency and Contracting Activity: The Louis Stokes Cleveland Department of Veterans Affairs Medical Center (VAMC), 10000 Brecksville Road, Brecksville, Ohio, 44141, VISN 10 Contracting Activity 90(C). Nature/Description of Action: Vendor shall provide all labor, supervision and parts necessary to correct critical and non-critical issues with the scope located in the GI Lab of the Louis Stokes Cleveland VAMC- Wade Park Campus. Non-critical failures are defects that are out of specification but do not represent a safety issue or significantly impact operability of the scope. Critical failures are defects that require repair. Critical defects are generally safety issues or defects which impact operability of the scope. The insertion tube and image are both critical issues. The buckling on the insertion tube can lead to further damage to the internal elements of the scope. The orange flare in the image does not allow a clear image. These two items combined would lead to having a full refurbishment done on the scope. The activity proposes to solicit the services of Olympus America Inc., at the Louis Stokes Cleveland VA Medical Center, to repair critical and non-critical deficiencies in the scope used in our GI Lab. This service is critical to the mission of the medical center to ensure the highest quality of care for our patients because without this service the GI Lab will have to cancel cases. Determination of a Fair and Reasonable Cost: Price is based on previous repair jobs done by this company the knowledge of the COTR. Pricing for these services are also the same that the "public" would pay. Additional Support for the use of other than full and open competition: Purchasing these services for Olympus America Inc. will ensure the scope in the GI Lab will be repaired with original equipment manufacturer parts (OEM).
