Description
The Clinical Center Rehabilitation Medicine Department located in Bethesda, Maryland proposes to modify Contract No. HHSN269200700002C dated September 28, 2007 "An Outcome Monitoring System for Post-Acute Care" with Kaiser FDN Research Institute, 1800 Harrison Street, 16th Floor, Oakland, California 94612 to add the following requirements: 1. Have the contractor examine the health and functioning of adults with stroke from 6 months post-stroke to 2 years post-stroke; 2) examine factors that affect declines or improvements in health and/or functioning in this cohort; and 3) to describe ongoing health care use and informal sources of support with emphasis on family care giving. The additional research will continue to follow the existing cohort of 200 adults established during the earlier phases of this contract until 2 years after their first acute stroke (the time of enrollment in the OMS-PAC study). Data being collected for the OMS-PAC study will continue to be collected at 12 months, 18 months, and 24 months post-stroke. These data elements include AM-PAC scores, social/role function instrument scores, and data elements available through Kaiser Permanente's existing databases which include socio-demographic information, occurrence of new (incident) conditions, and medication use. In addition, Kaiser Permanente's existing databases will allow collection of information on health care service use of the cohort over the 18 month period. Finally, a fixed form module capturing information on family care giving and informal sources of support will be added to the existing computerized data collection instrument. This modification will enable KFRI to continue this research without delay, advance the work, extend data analyses, and enhance the value of the study to the government and science. Kaiser FDN Research Institute is the only know n source with the capability to continue to provide the additional services described herein. In accordance with FAR 6.302-1, Kaiser Foundation Research Institute (KFRI) is uniquely qualified to provide these services because: they have sole access to the identity and contact information for the cohort of 200 adults with stroke enrolled in the current OMS-PAC study under the existing contract. Furthermore, implementation of the OMS-PAC study has led to effective methods of tracking participants over time, yielding exceptional follow-up rates and successfully capturing all necessary data elements. It is in the best interest of the Government to award this contract modification as a sole source because an award to any other source would result in substantial duplication of cost to the Government that is not expected to be recovered through competition, and would create unacceptable delays to the Government in completing this important research. This is not a request for proposals (RFP) nor is an RFP available. Submission of any information provided in response to this announcement is purely voluntary and the Government assumes no financial responsibility for any costs incurred. Responsible sources that believe they posess the require expertise and capabilities identified above may submit a response within 45 days from the posting of this announcement which will be considered by the Government. A determination by the Government not to compete this proposed requirement based upon responses to this notice is solely within the discretion of the Government.
