- Description
- RFP VA-528-09-RP-0139 is issued to provide primary care services at a location in Jefferson County in the State of New York. The Veterans Affairs Healthcare Network Upstate New York (VA) desires to enter into a contract for community based primary/preventive care services for veterans. The resulting contract will be for a one-year period with two one-year renewal options to commence 90 days providing services after date of award. The Contractor will be responsible to furnish medical facilities, clinical providers, equipment, supplies, routine radiology and administrative functions to fulfill the level of care specified in the RFP. Service will be at an all-inclusive capitated rate for primary care and mental health services. The NAIC code is 621498 and the Commercial items solicitation is unrestricted.
