Description

The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for a Registered Nurse to provide services at the Fort Yates PHS, IHS Hospital in Fort Yates, North Dakota and McLaughlin PHS Indian Health Center in McLaughlin, South Dakota. The period of performance will be from Date of Award through September 30, 2009 or 960 hours; whichever comes first. The Contractor will be working a compressed schedule; 8, 10, or 12 hour shifts. The Contractor may be scheduled to work holidays, weekends, and nights as determined by the Director of Nursing. The Contractor shall perform according to the Statement of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-09-085, shall be in writing either by email or fax. In the written request please reference RFQ number RFQ-09-085 and provide a postal mailing address or email address where the RFQ package can be sent. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be May 29, 2009. The closing date of the RFQ package is June 15, 2009. This is a small business set-aside.
