Performance Work Statement is attached. 7) Place of Contract Performance: 10th Medical Group, 4102 Pinion Drive, Air Force Academy, CO 80840 8) Dates of Performance: Base Year (1 Jul 09-30 Jun 10) Option Year 1 (1 Jul 10-30 Jun 11) Option Year 2 (1 Jul 11-30 Jun 12) Option Year 3 (1 Jul 12-30 Jun 13) Option Year 4 (1 Jul 13-30 Jun 14) 9) Service Acceptance will be done via Wide Area Work Flow. 10) Freight on Board: Not Applicable. 11) The provision at 52.212-1, "Instructions to Offerors -- Commercial", applies to this acquisition and a statement regarding any addenda to the provision. Addenda states the following: Submission of proposal price data and resumes as required below. The offeror must provide the following information to fulfill this requirement: --Monthly price for each of the following CLINs (0001, 1001, 2001, 3001, and 4001), extended price, total price per year, and overall total price. NOTE: CLINs 0002, 1002, 1003, and 1004 should reflect $100,000 (Not to Exceed) and added to each associated CLIN, to provide a total for that year. --Provide a staffing plan for the work requirement that depicts what positions will be staffed by the prime contractor and what positions will be staffed by subcontractors ( if any). --Provide resume(s) (curriculum vitae), contractor format is acceptable. --Please provide a letter of financial responsibility (on bank letterhead) and a point of contact who may verify the company's, group's, or individual's financial responsibility (include name, address, and phone number). --Provide a statement on how the "Limitations in Subcontracting" FAR 52.219-14 will be met. The statement shall state the following: By submission of an offer and execution of a contract, the Offeror/Contractor agrees that in performance of the contract in the case of a contract for Gastroenterology Physician Services or General Surgeon Services, at least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern. --Please submit the requested information no later than 29 May 2009 (Friday), 2:00 pm Mountain Standard time to the POC listed below. E-mail is the preferred method of proposal submission. (See Attached file named 52.212-1) 12) The provision at 52.212-2, "Evaluation -- Commercial Items", applies to this acquisition, and the specific evaluation criteria is included in paragraph (a) of this provision. Paragraph (a), fill in part of FAR 52.212-2 reads as follows: Basis of award: Award will be made using Lowest Price Technically Acceptable (LPTA) process. Resumes from Gastroenterology Physicians and General Surgeons will be received and evaluated against the physician qualifications stated in the Performance Work Statement (PWS) (i.e technically acceptable) and in attached checklist. Physicians may be a part of a group, company, or individual. Resumes will be determined as a "pass" or "fail". Award will then be made to a group, company, or individual on the basis of the lowest evaluated price of proposals meeting or exceeding the technical acceptability standards. Also, Technical acceptability, for relative importance, is the same as price. NOTE: Past performance is not being evaluated. Paragraphs (b) and (c) will be used and remain unchanged. (See Attached file named 52.212-2) 13) Offerors shall include a completed copy of the provision at 52.212-3, "Offeror Representations and Certifications -- Commercial Items", as applicable, with its offer. (See Attached file named 52.212-3) 14) The clause at 52.212-4, "Contract Terms and Conditions -- Commercial Items", applies to this acquisition and a statement regarding any addenda to the clause. (See Attached file named 52.212-4) 15) The clause at 52.212-5, "Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items", applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. (See Attached file named 52.212-5) 16) Additional contract requirement(s) or terms and conditions: An incentive fee is associated with this contract. The incentive fee will be accomplished as follows: The incentive fee will be in the form of a quarterly performance incentive or penalty. A performance incentive or penalty will be assessed against the contractor's earnings at a rate of $1,000 for each increment of 8 encounters and procedures above or below the 660/quarter minimum. There will be a ceiling (860 total encounters and procedures/quarter) and floor (460 total encounters and procedures/quarter) established in the contract. The maximum amount of fee available to earn is $100,000/year. 17) The Defense Priorities and Allocations System (DPAS) is not applicable to this acquisition. 18) The date, time and place offers are due: Please submit the requested information no later than 29 May 2009 (Friday), 2:00 pm Mountain Standard time to the POC listed below.
