Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number W81K00-09-T-0159 is issued as a request for quotation (RFQ) St Jude Medical or Equivalent. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32. This acquisition is being conducted as a commercial item acquisition in accordance with FAR Part 12. This requirement is unrestricted and all responsible sources may submit a quotation, which will be considered under NAICS code 334510 and the small business size standard is 500 employees. The requirement in this solicitation: CLIN 0001, EP WorkMate Computerized Recording System and Workstation Qty 1 Each Sub components are listed below: Item WM-01-1203 EP WorkMate 120 Item MM-01 MapMate Interface Item WM-LM//EP LinkMate Item WM-NET-WS WorkMate Networking Interface CLIN 0002 Ensite Mapping System, QTY 1 Each CLIN 0003 Ensite Verismo Segmentation Tool, QTY 1 Each CLIN 0004 Ensite Fusion US field install Kit, QTY 1 Each Salient characteristics are: CLIN 0001, EP WorkMate Computerized Recording System and Workstation components are: Acquisition of unipolar signals at acquisition rate of 1,000 samples/sec or higher; must interface with Carto (Biosense Webster) system; interface with current RF generator (Stockert, Biosense-Webster), Fully integrated digital stimulator; Provide image import for intracardiac echo (ICE), fluoroscopy, other images (xVGA or HDI video formats); Post-case review of all signals from workstation outside EP lab. CLIN 0002 Ensite Mapping System 3D mapping system must offer choice between contact (via NavX patches), non-contact (64-electrode balloon catheter), System built with open system philosophy, allowing cardiologist choice of any catheter available; Data collection at rate of 400 samples/sec, providing high-resolution anatomical model of the heart CLIN 0003 Ensite Verismo Segmentation Tool quickly perform segmentation of a patients cardiac anatomy; Simple point and click interface; compatibility with DICOM3 data sets with any CT or MR scanner; Visualize to 16 anatomic structures CLIN 0004 Ensite Fusion US field install Kit, 3 D Mapping functions, display all electrical information and labeling 52.212-1 [Instructions to Offerors]; Addendum 52.212-1; Federal Acquisition Regulation (FAR) provision 52.211-6 Brand Name or Equal applies to this acquisition. Vendors shall comply with FAR provision 52.211-6 if submitting other than the brand name products. Award will be all or none. The following is incorporated full text for FAR 52.212-2 [Evaluation-Commercial Items]: a. The Government will award a contract resulting from this solicitation to the responsible contractor whose quote, conforming to the solicitation, is Lowest Priced, Technically Acceptable (LPTA). Award will be on an all or none basis. The following factors will be used to evaluate quotes: Technical acceptability and price. 1. Technical acceptability: If providing an equal item(s), comply with FAR Clause 52.211-6, Brand Name or Equal. Explain how your company will meet the salient characteristics of the items as specified in the solicitation. Prospective contractors shall demonstrate that the system offered complies with the technical requirements described in the salient characteristics by submission of a written capability statement with their offer. 2. Price: All quotes will be evaluated on Technical Acceptability. The award decision will be based on the Lowest Priced, Technically Acceptable quote. 3. A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offerors specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipts, unless a written notice of withdrawal is received before award.
