- THIS IS A NOTICE OF INTENT, NOT A REQUEST FOR A PROPOSAL. A SOLICITATION DOCUMENT WILL NOT BE ISSUED AND PROPOSALS WILL NOT BE REQUESTED. The National Heart, Lung, and Blood Institute (NHLBI), Office of Acquisitions (OA), for the National Institutes of Health, http://www.nhlbi.nih.gov, intend to award a purchase order on a noncompetitive (sole source) basis to Illumina Inc., 9885 Towne Centre Drive, San Diego, CA 92121-1975 to purchase Item (1) Premium – Genome Analyzer Extended Warranty, Catalog #SUV-301-1004, Serial Number 300434; (2) Premium – Cluster Station Extended Warranty, Catalog #SUV-302-1004, Serial Number 0140; and (3) Premium – Paired End Module Extended Warranty, Catalog #SUV-303-1004, Serial Number 200293 for the period covering September 2, 2009 through September 1, 2010. The service agreement shall include: Item (1) Includes full coverage on parts and labor with two preventative maintenance visits per year, 48-hour average on-site response time, demos, training, non-critical and critical updates, unlimited web-ex, 1-3 day in-house applications training. The maintenance service must be performed by Certified Engineers from Illumina, Inc. Item (2) Includes full coverage on parts and labor, two preventative maintenance visits per year, 48-hour average on-site response time, demos, non-critical and critical updates, unlimited web-ex, 1-3 day in-house applications training. The maintenance service must be performed by Certified Engineers from Illumina, Inc. Item (3) Includes full coverage on parts and labor, two preventative maintenance visits per year, 48-hour average on-site response time, demos, non-critical and critical updates, unlimited web-ex, 1-3 day in-house applications training. The maintenance service must be performed by Certified Engineers from Illumina, Inc. The sole source determination is based upon the fact that the equipment is manufactured by Illumina, Incl., and only the manufacturer is authorized to provide maintenance service. This acquisition is being conducted under FAR Part 13, simplified acquisition procedures. The North American Classification System (NAICS) code applicable to this requirement is 811219 and the associated small business size standard is $7.0 Million. The Small Business Set-Aside does not apply. The service contract is for a twelve-month period. This notice of intent is not a request for competitive proposals. Interested parties may identify their interest and capabilities in response to this requirement. The determination by the Government not to compete the proposed contract based upon responses to this notice is solely within the discretion of the Government Information received will normally be considered solely for the purpose of determining whether to conduct future competitive procurement Responses to this announcement, referencing synopsis number NHLBI-PB-(HL)-2009-143-DDC, may be submitted to the National Heart, Lung and Blood Institute, Office of Acquisition Center, Procurement Branch, Quotations must be submitted in writing to the National Heart, Lung and Blood Institute 6701 Rockledge Blvd., Room Suite 6100, Bethesda, Maryland 20892, Attention: Deborah Coulter, Emails will be accepted. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency.
