This is a USPS Highway Transportation Contract The following information will be needed to complete forms that will be returned with your proposal; HCR # 41538 Solicitation # 150-97-09 Date of Solicitation – 05/05/2009 Begin Contract Date - 07/01/2009 End Contract Date - 06/30/2011 For Mail Service Between City, St – Pikeville, KY City, St – Phyllis, KY Annual Hours – 1,565 Annual Miles – 33,459.0 Required Vehicle – Straight Truck with 1,000 cubic foot capacity cargo compartment Attachment labeled " Forms Contractor must Complete and Return" will need the above information and be fully completed by the contractor. Completed forms will need to be sent to the following address; EASTERN AREA OFFICE SOLICITATION NO. 150-97-09 5315 CAMPBELLS RUN RD PITTSBURGH PA 15277-7070 Contractor must make sure to include the Solicitation number in the address. All documentation for proposal must be received by the above address no later than June 2, 2009 by 1500 pm.
