Description
The purpose of this amendment is to clarify the Government's requirements for the PBX telephone system upgrades at the Fort Washakie and Arapahoe Health Centers. Basic information about the telephone systems: 1. Telephone Model - Lucent Technologies Definity; 2. T1/PRI at each Location - One per location, Ft. Washakie Clinic and Arapahoe Clinic; 3. Number of Telephones - Ft. Washakie Clinic has 86 telephones and Arapahoe Clinic has 37; 4. One Voice Mail per Location - Each clinic needs an Avaya Communication Manager upgrade with auto attendant functionality; 5. Site Survey - A visit can be scheduled anytime on Monday - Friday, 8:00 am to 4:45 pm. The Government's points of contact are Paul Lawson and Beaver C'Bearing. The Agency does not have a Scope of Work for software licensing or hardware requirements; Needs identified by the facilities: 1. One system for both clinics, with direct dial by extension, to either clinic; 2. Voice mail for all extensions, with capability to change phone Message, to have several different messages, or to have no Messages; voice mailbox is integrated with Microsoft Outlook; 3. Auto-attendant who answers calls initially, routes to extensions, with no more than 4 options for callers; 4. Ability to put system on evening mode, so answering service can pick up calls; also message to all callers with this mode; 5. Quality phone sets that can be desk or wall phones, have speakerphone, mute, transfer/park,time of day, caller ID, 8 lines, conference capabilities (we use Avaya 6400 series telephones, with 8 lines, 16 lines, etc). Our main reception areas have a bigger set, with an additional 24 buttons); 6. Additional capabilites to Fax from PC, thru phone lines; 7. Additional phone and Fax lines (6-10 lines, 3-4 faxes); 8. Capability for "call center" in the future; 9. Ability for one clinic to receive all calls for both facilities so that on Sunday Clinics the clinic that is open can take all calls; and 10. Capability for various reports to monitor systems, security logs user logs, etc. All other terms and conditions remain unchanged.
