- 1) Type of Notification: Sources Sought This is a sources sought synopsis only. The purpose of this notice is to gain knowledge of potential qualified sources. Responses to this notice shall include the following: (a) company name; (b) address; (c) point of contact; (d) "number of employees" as defined by the Federal Acquisition Regulation (FAR) part 19.101; and (e) the amount of "annual receipts" as defined by the Federal Acquisition Regulation (FAR) part 19. In addition, the respondent must provide a capability statement that addresses the organization's qualifications and ability to perform as a prime contractor the work described below. 2) Description: Personal Services (Family Practice Physicians - 8 FTEs) The following is a brief summary of the primary duties that the Family Practice Physician will perform. This list is not all inclusive: Shall maintain a program of comprehensive health, to include preventive medicine, behavioral sciences, and community health. The HCP shall examine patients, formulate differential of diagnostic plans, define and order required diagnostic testing, interpret examination findings and test results and implement treatment plans. The HCP shall diagnose and treat a wide-range of pediatric and adult acute and chronic diseases and injuries. Prepares records and reports as required in support of services ensuring for correctness of all prepared and transcribed reports. Provides immediate evaluation and management of emergency problems. Documents history, physical examinations, required or recommended laboratory and imaging examinations and diagnoses. 3) Qualifications: The HCP shall have provided primary care services for a minimum of 36 months which may include residency training within the past 48 months. The HCP shall also be Board Eligible and/or Board Certified. 4) Place of Performance: Locations include, but are not limited to: Wilford Hall Medical Center (WHMC) at Lackland AFB; Brooke Army Medical Center (BAMC); 37th Medical Group (MDG); Brooks City-Base Clinic; Kelly; North Central Federal Clinic; Reid Trainee Health Clinic; and any successor organization(s) of WHMC or 37 MDG, if applicable.
