Description
The South Dakota Army National Guard has a requirement for Soldier Periodic Health Assessments (PHA) and supporting lab tests. The health assessments are required on location in Parkston, SD on 1-3 May 2009. The contractor is required to provide their own Mobile Medical Units for conducting exams onsite. The contractor is required to be HIPPA qualified and enter assessment data (CACC Access) into MEDPROs. The request for proposal and requirement details are listed within W912MM-09-T-0022 solicitation Performance Work Statement and attachments posted at: https://sdguard.ngb.army.mil and then by selecting contract solicitations. The Government will evaluate each prospective offer utilizing proposed costs for assessments and tests based on the tentative numbers provided. The contractor must be found acceptable based on the factors listed in Clause 52.212-2 in the Solicitation. The contractor will send in a list of its team members and identify their qualifications. No award shall be made unless the Contracting Officer makes confirmation of affirmative determination of responsibility for the potential contractor. Experience or knowledge completing Army PHAs is required. Potential contractor must complete the Central Contractor Registration process prior to award of contract. The applicable NAICS is 621399 for Miscellaneous Health Services. It is anticipated that the solicitation will be available at this website o/a 14April09 Tentative RFP solicitation response date is 22April09.
