Description
The Federal Bureau of Prisons, Field Acquisition Office in Grand Prairie, Texas intends to make award for the provision of Phlebotomy services on behalf of the Federal Medical Center (FMC) Carswell located in Fort Worth, Texas. Faith-based and Community-based organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. The anticipated date of award will be approximately June 15, 2009. The contract performance period is anticipated to consist of a base year from effective date of award through 12 months from effective date of award, and will include four, 12-month option years for renewal. Contractor shall provide professional phlebotomy services to inmates. The Contractor shall be responsible for, but not limited to, collection of blood and urine samples, preparation and tracking of all lab samples, and daily maintenance of laboratory equipment, including advising the Contract Monitor when equipment requires professional maintenance repair; biological specimen preparation and packaging for shipment to contract laboratories, maintenance of laboratory logs and other records as necessary to comply with Clinical Laboratory Inspection Amendment (CLIA) guidelines; informing the Contract Monitor of all information pertaining to the services rendered, i.e., specimens obtained; and reporting any problems arising in regards to laboratory services. Normally the duties of the contractor would not require direct entries into the inmate's medical record. However, some information generated by the Contractor may be included in the inmate's medical record. Services will be provided in the method as considered normal in the local community and in accordance with Joint Commission standards and Bureau of Prisons policies. The phlebotomist must either have completed a 12 week Lab Technician program, an approved state program or possess an associate's degree. The Contractor must have at least six months experience as a phlebotomist in a clinical setting. The Contractor shall provide written documentation of certification (copy of all diplomas and/or certificates) that permits them to perform services in this statement of work for the duration of the contract. It is estimated 30 one (1) hour sessions per week will be required for phlebotomy services. The number of sessions per week will not exceed 30 hours per week. The anticipated schedule is Monday through Friday between 7:30 a.m. and 3:30 p.m. excluding Federal holidays. This schedule may be adjusted to meet the needs of the laboratory. Services will be performed in the Laboratory Services Department at the Federal Medical Center (FMC), Carswell located at J Street, Building 3000, Fort Worth, Texas 76127. The solicitation will be made available on or about May 4, 2009 and will be distributed solely through the General Service Administrations Federal Business Opportunities website at http://www.fedbizopps.gov/. Hard copies of the solicitation will not be available. The site provides downloading instructions. All future information regarding this acquisition, including solicitation amendments, will also be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. All contractors doing business with the Federal Government shall be registered in the Central Contractor Registration (CCR) database. The website for registration is www.ccr.gov. The solicitation is a total small business setaside. The North American Industry Classification System code for this solicitation is 621399, with a size standard of $7.0 million. The anticipated date for receipt of quotes is June 4, 2009; however, this is a projected estimate and quoters should refer to Page 1 of the solicitation (Block #8) for the actual date quotations are due. All responsible sources may submit a quote which will be considered.
