- Attachment 4 to SF1449 Past Performance Survey Sheet Attachment 3 to SF1449 List of Reference (Sample format) Attachment 2 to SF1449 Performance Plan Attachment 1 to SF1449 Performance-based Work Statement SF1449 Notice to offeror RFQ Cover letter "Memorandum for Prospective proposer" Description: The Contractor shall provide all personnel, equipment, tools, materials, supervision and other items or services necessary to ensure that transportation of Government Mail Containers (All Nippon Airway's UDLs). The transportation is between Hiroshima Airport and United States Marine Corps Air Station, Iwakuni via Sanyo Express way and performed in accordance with applicable Japanese laws and trade practices. The period of performance is anticipated to be for a one-year basic period with four (4) one-year options, for total five (5) years. The anticipated performance start date is 1 Oct 09. Offeror will propose a rate for transportation by trucks capable of transporting four and/or six containers. The transportation rate includes but not limited to tolls, licenses, costs of overtime, holiday work, or for operations during stormy and or inclement weather. Award will be made to the offeror whose offer represents the best value to the U.S. Government considering the evaluation factors stated in the solicitation. The solicitation is not restricted. The due date for responding to the Market Research Questionnaire is 16 Jan 09. The solicitation will be available on this web site and will be issued to all firms who respond in writing to this synopsis. A pre-proposal conference will be scheduled at Yokota Air Base, Japan. Details will be posted when determined.
