Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W90EGT590561001 is issued as a Request for Quote (RFQ). This request for Quote is due by 10 APR 2009, 12:00pm (Pacific Time). The RFQ will result in a firm fixed price contract. This solicitation is 100% set aside for small business. The North American Industry Classification System (NAICS) code for this acquisition is 624410. This acquisition is under Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items and Part 13.5, Test Program for Certain Commercial Items.. This requirement is for: SERVICES NON-PERSONAL: Furnish all labor, tools, equipment, material, transportation and supervision to: PROVIDE CHILDCARE SERVICES IN ACCORDANCE WITH THE STATEMENT OF WORK FROM 24-26 APR 09 AT THE HYATT HOTEL LOCATED AT BELLEVUE, WA FOR THE WASHINGTOON NATIONAL GUARD VOLUNTEER & REUNION WORKSHOP. Statement of Work Childcare Provider Support Washington National Guard Volunteer & Reunion Workshop 1. Non-personal services- The Washington National Guar will be requiring comprehensive childcare for several age group children of attendees for the Washington National Guard Volunteer & Reunion Workshop taking place at the Hyatt Regency Bellevue, 900 Bellevue Way, Bellevue, WA on 24-26 April 2009. 2. REQUIREMENTS: 2.1The contractor will be responsible for the complete set up on-site and be able to be self-sustained throughout the event located at the Hyatt, Bellevue. 2.2Personnel/caregivers must be experienced childcare professionals, certified in CPR and First Aid, fingerprinted with background checks. The servicing agency shall have copies of all certifications, fingerprint clearances, detailed resumes, and have reference checks and security background checks on each staff member. A copy of the certifications and verified background checks for all staff serving at this event shall be provided NLT 20 April 2009 to our POC (name provided upon award of contract). If staff changes occur after 20 April 2009, the servicing agency will provide a copy of the same documentation for the new staff prior to their working at the event. 2.3The servicing agency will provide all the required staff training. The staff will provide the POC with a list of planned schedule of activities, children attending and emergency procedures specific to the event location to the POC 3 days prior to the event taking place. On-site childcare permits the maximum attendance of the family at these vital information sessions. 2.4The following chart provides detailed services required on the specific dates and age groups. The contractor will be responsible for providing the required number of personnel regarding the student provider ration that meets the governing laws in the State of WA. This will allow the contractor to plan accordingly. NOTE: The guaranteed minimum the contractor will be paid for will be 10 children for each day of required services not-to Exceed 30 children. The govt will provide updates of the children registrations once the contract is awarded until the final update on 15 April 2009. Note: The evening session on 25 April 2009 will have the full range of children in age up to 12 years as the concurrent Youth Symposium is closed during these hours. The numbers per age group will be provided on 15 April 2009, this will ensure the contractor provides the required age group child/provider ration. 2.6 Safety and Security: For the Safety of the children, bracelet and photo identification check-in/out must be provided to ensure full control of custody, plus details of each childs dietary and medical needs. Children must be continually accounted for and an audit of equipment and supplies will be performed at the end of each day. 2.7 Liability Insurance: The servicing agency will carry sufficient corporate liability insurance to cover possible claims, and an insurance rider to protect us as the client. Proof of this insurance will be provided to our POC by 20 April 2009. 2.8 Experience: Selected Company should have at least five years company history in childcare industry. Current listing in the Central Contractor Registration (CCR), a primary business registrant database for the U.S. Federal Government is a plus factor but not a requirement for award purposes. 2.9 Supplies and Activities: The contractor will provide as a minimum themed events, supplies and activities to the specific age group. These supplies will be provided at the facility. The contractor will be responsible for their own supplies to include shipping costs for any supplies and materials being utilized at the facility. Contractor will provide age-specific room decorations for each event; and incorporate customized theme-based schedule of child- and pre-teen-care programs. Additionally, the contractor will provide infant and toddler supplies including changing station, cribs and/or beds, and infant safe toys during the 3 day event. The contractor will be responsible for the set-up of the childcare room/rooms prior to the event times. Rooms will be provided with enough time to set-up by the Hyatt hotel at no cost to the contractor. The facility and basic room equipment (chairs and tables) will be covered by the Government. Note: the contractor will be allowed to leave their equipment and supplies at the end of the event day and/or till the next event takes place. 2.10 The parents will be responsible for providing their own diapers, formula, food, snacks, drinks, clothing, and blankets for their own children at time of drop-off or as appropriate throughout the day. 2.11 Cleaning of facilities: The contractor at the end of the shift will perform general clean-up and pack-up procedures. The room/rooms and equipment will be cleaned in accordance with the State of WA sanitary conditions standards at the end of each session in readiness for the next day or event. On the last day of the event 26 Apr 09, the contractor will ensure all rooms are left in the same condition as they were accepted from the venue staff on 24 April 2009. 2.12 Reports and Documentation: The contractor will create a report that includes daily attendance, collected evaluations and a digital photo CD that incorporates all activities that occurred during the even(s) t. All reports will be delivered together with the final invoice or at the end of the event for the keeping of WA National Guard. Payment will not be made unless these are submitted.
