- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 090001 and is issued as a Request for Quote (RFQ). The NAICS code is 423450 and the size standard is less than 100 Employees. The solicitation document and incorporated provisions and clauses are those in effect though Federal Acquisition Circular 2005-30, Effective 17 Feb 2009. The proposed contract is 100 percent set aside for small business concerns. The acquisition is a firm fixed priced purchase order in support of the Montana Army National Guard J5/7 office. The J5/7 is requesting expendable medical supplies for an upcoming mission and combat lifesaver course. ALL supplies SHALL be in Montana no later than 13 April 09. This solicitation is waiting for funding. The government is seeking an offeror who can provide all medical supplies. If any offeror is not able to quote all the supplies due to the condensed time frame, please quote the supplies available for immediate purchase and identify those items you can not provide a quote for. Please provide technical specification, warranty information, and discount information if a quantity discount applies. The provision of FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (Feb 2009) applies to this RFQ. 52.212-3, the clause needs to be completed and a signed copy of this provision shall be submitted with any offer. 52.212-2, Evaluation, Commercial Items (Jan 1999). The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Price, Past Performance and Technical Capability. Past performance and technical capability, when combined, is less important when compared to price or cost. Vendors are requested to submit past performance information to include persons of contact on their last two contracts for similar products.
