- Offeror Representations and Certifications Past and Present Performance Questionnaire Wage Determination Description of Services Statement of Work SUBJECT: Request for Quotation - Barber Shop Concession at Dover AFB, Delaware, Solicitation Number DOVEF220890016BarberShop 1. Dover AFB is requesting quotations from any business interested in providing the Barber Shop Concession service at Dover AFB, Delaware. The contract start date is 19 July 2009, for a one (1) year period. The contract will include four (4) renewable/option years for a total contract lifespan of five (5) years. 2. If you wish to compete for the opportunity to provide this service, please submit a package not later than 3 April 2009, 4:00 PM Eastern Time. Please note that the successful contractor must comply with all elements of the statement of work (SOW) (Attachment 1). 3. Your package must include the following. All items must be completed and provided to be considered fully responsive/eligible for award: a. A cover sheet with the solicitation number DOVEF220890016BarberShop, your name, the company name, address, phone number, fax number, e-mail address, and DUNS number. b. A completed Solicitation to include Article I-IV, filling in the blanks on pages 1-3. Note that the NAF General Provisions, along with the completed Articles, comprise the basic Concessionaire contract document. c. A completed Description of Services, (i.e. a list of the prices your company will charge for services provided) (Attachment 2). Note: Only the services on the Description are solicited or required at this time. In accordance with the SOW, expanded services may be offered after contract award. d. Completed Offeror Representation and Certifications FAR 52.212-3 (Attachment 5) e. A copy of current State licensure/certification card. 4. The contractor will send the Past and Present Performance Questionnaire (Attachment 4) to no more than three (3) companies for which they have done similar work. The past performance will be assessed based on the ratings detailed in the questionnaire (Exceptional, Good, Satisfactory, Marginal, Unsatisfactory, and Neutral). The questionnaire must come directly to Contracting from the reference company; questionnaires will not be considered if received from the offeror. 5. A site visit will be offered to all interested parties on 27 March at 10:00 AM Eastern Standard Time (EST) at the Barbershop, which is located within the Collocated Club (Landings) on Dover Air Force Base. If you are interested in attending the site visit, please send the full name of all attendees, along with driver's license numbers with state of issue, to Amy Burke at the above referenced email address. You must submit the names to Mrs. Burke no later than 4:00 PM EST on Tuesday, 24 March 2009. Please include a cell phone number for a cell phone that will be carried on the 27 March 2009 site visit date. 6. All quotes must be sent via email to amy.burke@dover.af.mil, or faxed to (302) 677-2309, ATTN: Amy Burke, 436 Contracting Squadron, Bldg 639, Room 242, Dover AFB, DE 19902-5639. Email is the preferred method to provide quotations. Quotes must be signed by a responsible officer of the company. 7. Please reference FAR 52.212-1. Per FAR 52.212-2, an award will be made to the contractor who is determined to offer the best value to the government based on price and past performance. Price is approximately equal to past performance when being evaluated. Central Contractor Registration (CCR) registration must be current in order to receive an award. 8. The Solicitation, which will become the contract after annotation by the successful offeror, to include Articles I-IV (3 pages), Special Provisions (2 pages) and NAF General Provisions (6 pages) follows.
