Description
In accordance with VHA Handbook 1140.6 the VISN 19 Rocky Mountain Network is soliciting using full and open competition and commercial items procurement procedures to provide Home Healthcare, Hospice and Respite care services to eligible and enrolled veterans of the VISN 19 Rocky Mountain Network. VA anticipates multiple award of Basic Ordering Agreements (BOA's) to satisfy this requirement. Services are sought for all counties under the jurisdiction of the VISN 19 Rocky Mountain Network to include all counties within the jurisdiction of the following medical centers; the Eastern Colorado Health Care System, Denver, CO; VA Medical Center, Grand Junction, CO; the Salt Lake City Health Care System, Salt Lake City, UT; VA Montana Health Care System, Fort Harrison, MT; VA Medical Center, Sheridan, WY; and VA Medical Center, Cheyenne, WY. Contractors may provide care to patients in all or a specified geographical portion of the following states: Colorado, Utah, Nebraska, Idaho, Kansas, Nevada, Wyoming, and Montana. The solicitation lists all counties under the jurisdiction of VISN 19. Services will be in accordance with VA polices and procedures and the regulations of the medical staff bylaws of the VA facility for which service is provided. Services will be required for a base year with four one-year pre-priced options for renewal. Offers may provide discounts from prevailing Medicare rates, however, in no circumstance shall price exceed prevailing Medicare LUPA rates. Services will include skilled and unskilled home health service, home infusion therapy, and routine home hospice care. Contractor shall be required to address the following evaluation criteria, which must be submitted as part of their proposal and shall address the following: 1) Offerors qualifications to include: a) qualification background and experience in providing these services to include a list of staff and a copy of their credentials; b) offerors knowledge and ability to interpret all applicable VA standards; c) Offerors current applicable State and/or Medicare licensing or certifications or proof that such documents are being processed to be submitted; e) fiscal capability and company stability to successfully comply with contract requirements to include background, experience, etc; f) personnel capability to compensate for personnel turnover during the period of the contract; g) Key personnel; h) qualifications and ability of all proposed subcontractors as relevant to all of the evaluation factors herein: 2) Past Performance and History of Practice: 3) Quality Assurance System: 4) Cost/price: 5) Utilization of Small and Small Disadvantaged Businesses. BOA's may be awarded with or without discussions resulting from the solicitation to the responsible offeror(s) whose proposal conforms to the solicitation that will be most advantageous to the Government, price and other factors as outlined in the solicitation considered. The NAICS code is 621610. The small business size standard is $12.5 million. All other requirements are included in the solicitation, which shall be posted on the FEDBIZOPPS website for downloading. Paper copies of the solicitation will not be issued and telephone requests or fax requests for the solicitation will not be accepted. The solicitation will be available for download at no cost at www.fedbizopps.gov. We anticipate the solicitation to be issued on or about March 16, 2008 however this date may change and we invite all interested parties to check frequently for a possible change to this date.
