- Attachment 4 Past Performance Questionnaire Attachment 2 Collective Bargaining Agreement Attachment 2 Wage Determination Attachment 1 Performance Work Statement EXHIBIT A Solicitation SYNOPSITATION FOR ELEVATOR MAINTENANCE FA8601-09-R-0001 This is a combined synopsis/solicitation for elevator maintenance, inspection, and repair of elevators to be procured for Wright-Patterson Air Force Base. On-base workload would include providing inspection, maintenance, and repair for approximately one hundred and nine (109) lifting products. The different types of lifting products consist of hydraulic elevators, traction elevators, and dumbwaiter elevators. This is a commercial service prepared in accordance with (IAW) the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. Due date for proposals is 6 Apr 09, 10:00 am EDT. Proposals should be submitted to 88 CONS/PKBB (Capt Joseph Megery), 1940 Allbrook Drive, Room 109, Wright-Patterson AFB OH 45433. Solicitation FA8601-09-R-0001 is a Request for Proposal (RFP). This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is attached at www.fbo.gov. Written solicitation is being used for ease in proposal preparation. Offerors should pay close attention to the instructions in Addendum to 52.212-1, Instructions to Offerors, which is contained in the solicitation document. Required contents of a proposal package are described in Addendum to 52.212-1. All applicable provisions and clauses are listed in the solicitation document. No hard copies will be sent. All documents must be downloaded from www.fbo.gov. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-29 effective 19 January 2009. Proposed acquisition is unrestricted, full and open. The associated North American Industrial Code (NAIC) is 238290, size standard is $14,000,000. An Indefinite Delivery/Indefinite Quantity contract is anticipated with a period of performance of 01 July 2009 through 31 December 2009 with a potential four (4) one-year option periods for a total of 60 months. Performance Work Statement (PWS) is attached at www.pixs.wpafb.af.mil. A registration page is attached to the web site. If you would like to receive notice of any amendments and/or addendum that may be issued to the solicitation, you may register. If you do not register, it is your responsibility to review the web site for any changes to the RFP. Contractors that are debarred, suspended or proposed for debarment are excluded from receiving contracts. Proposals prepared by debarred, suspended or otherwise ineligible contractors are at the risk and expense of the contractor. Contract financing will not be provided for this acquisition. Prospective contractors must be registered in the DoD Central Contractor Registration (CCR) prior to award of a Government contract. Offerors are further advised that failure to register in the CCR database will render your firm ineligible for award. Registration requires applicants to have a DUNS number. DUNS/CCR website is http://ccr.gov. Recommend registering immediately in order to be eligible for timely award.
