- Quotation Coverpage Combined Solicitation The Combined solicitation is also available in the attached Microsoft Word Attachment. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information include in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. The Request for Quote Number/Reference Number is: OM-CNCS-09-BL0305. This solicitation and incorporated clauses/provisions are those in effect through Federal Acquisition Circular 05-30 effective January 15, 2009. The solicitation incorporates by reference the following clauses and provisions: Federal Acquisition Regulation (FAR) 52.204-7; 52.212-1; 52.212-4; 52.223-6; 52.232-17; 52.232-25; 52.233-1; 52.233-3; 52.239-1; 52.243-1; 52.249-4; and 52.249-8. The solicitation incorporates in full text the following clauses and provisions: 52.212-2; 52.212-3 52.252-2; 52.217-8; 52.217-9; 52.232-18; 52.232-19; 52.212-5; (The following clauses listed in 52.212-5(b) and (c) are included by reference: 52.219-6; 52.222-3; 52.222-21; 52.222-26; 52.222-36; 52.225-3; 52.232-34; and 52.222-41 The Corporation for National & Community Service intends to solicit and award a single Firm-Fixed-Price, (Not-to-Exceed) purchase order for the effort under Federal Acquisition Regulation (FAR) Part 12 and 13. This is a full and open (Unrestricted) set-aside. The North American Industrial Classification System (NAICS) code for this requirement is 561450. Evaluation for award shall be Lowest Cost Technically Acceptable. The Government reserves the right to award the contract based on initial offers received, without discussions or negotiations of such offers. Therefore, it is important that each offer fully address the requirements stated in this combine solicitation This combined solicitation will close on Monday, March 30, 2009 11:00am Eastern Time. This requirement is for: Line Unit of Unit Total Item Description Quantity Issue Price Price 0001 Lost Alumni 175,000 Each $______ $_________ Recovery Services Not-to-Exceed (Deliverable shall be incorporated within pricing submitted for this CLIN) (See Statement of Work below) Period of Performance: From Date of Award to June 19, 2009 Instructions, Conditions, and Notices to Offerors CONTENT AND FORMAT OF SUBMISSION (1) A quotation submitted in response to this solicitation must be submitted electronically and be comprised of the following Sections: Section 1: Corporate Overview 1. Company's Professional History in relation to Statement of Work 2. Company's Facilities and Staff in relation to Statement of Work Section 2: Research Analyst Qualifications (All below items must be in relation to Statement of Work) 1. Company's Professional Research Analyst Qualifications 2. Research procedures (Procedures and methods of collecting information as prescribed within the Statement of Work) 3. Company's Overall Technical Capabilities 4. Software and Programs utilized to collect research information 5. Company's Security plans to secure personally identifiable information (PII) a. ensure information systems prevent and defend against unauthorized access to systems, networks, and data b. ensure the rigorous application of information security/ information assurance policies, principles, and practices in the delivery of all services 6. Key Personnel (Provide resumes representative of experience collecting information within statement of work). The resumes must be indicative of the following: a. Performing routine assignments in the area of research analyst b. Define data requirements and specifications c. applicable software principles and methods d. data management concepts and methods e. IT security principles, methods, and tools Section 3: Past Performance/References 1. Past Performance/References shall be similar in scope, size, and dollar value of this RFQ. Past Performance/References must be submitted for current work being performed or work that has been done within the past two years. Section 4. Pricing Quotation **Offers that are received without addressing the Section information as prescribed above will be deemed non-responsive and will not receive consideration for award. ELECTRONIC SUBMISSIONS ALL OFFERORS MUST BE SUBMITTED ELECTRONICALLY U.S. Postal Service, Over Night Carrier or Fax submissions shall not be an acceptable form of submittal. All required information shall be sent via one email. Place the RFQ number within the subject line of your submittal. Electronically submitted proposals shall be sent to the following email address: blorthridge@cns.gov Brandon Lorthridge, Contracting Officer **It is the contractor's/offerors responsibility to ensure that electronically submitted quotations are received by the Government by the due date.** (2) Any data previously submitted in response to another solicitation will be assumed unavailable to the Contracting Officer; and this data must not be incorporated into the technical offer by reference. (3) Clarity and completeness of the offer are of the utmost importance. The offer must be written in a practical, clear and concise manner. It must use quantitative terms whenever possible and must avoid qualitative adjectives to the maximum extent possible. (4) Offers must be legible, double spaced (personnel resumes may be single spaced), typewritten (on one side only), in a type size not smaller than 10 point pitch with a one-inch margin on all sides, on paper not larger than eight and a half by eleven inches and not exceeding the page limits established in this solicitation. Pages in excess of the individual limitations shall not be read, and the offer shall be evaluated as if the excess pages did not exist. QUESTIONS PERTAINING TO SOLICITATION (1) All questions shall be addressed to the Contract Specialist at the following email address: Brandon Lorthridge, Contract Specialist blorthridge@cns.gov Please send all questions via email. Questions will not be taken or answered over the phone or by fax. Please include the Request for Quotation (RFQ) Number in the subject line. Once questions are compiled, they will be answered by an issued amendment to the solicitation. (2) Questions will be permitted from March 5, 2009 through March 17, 2009. Questions submitted after March 17, 2009 11:00am Eastern Time shall not be answered. REQUIRED SUBMITTAL ITEMS THAT MUST ACCOMPANY QUOTATION Offeror shall complete Quotation Cover page at Attachment 1 in its entirety, submit Current ORCA Record or FAR 52.212-3 (Offeror Representations and Certifications -Commercial Items) in its entirety, and submit with quotation. **Offerors that submit quotations without the quotation cover page, ORCA/FAR 52.212-3 will be deemed non-responsive and will not receive consideration for award. Statement of Work VISTA Lost Alumni Recovery Project Background The Corporation for National & Community Service (CNCS), is a federally owned corporation. Volunteers In Service To America (VISTA) is a program that is a part of CNCS. In partnership with state and local government and non-profit organizations, we administer National Service programs, including AmeriCorps, Senior Corps and Learn and Serve. The CNCS mission is to engage Americans of all backgrounds in community-based service. This service addresses the nation's education, public safety, human and environmental needs to achieve direct and demonstrable results. In doing so, the Corporation fosters civic responsibility, encourages community teamwork, and provides education opportunity for those who make a substantial commitment to service. Since 1964, more than 170,000 Americans served through VISTA. Today, nearly 6,000 VISTA members serve over 1,200 projects throughout the country-working to fight illiteracy, improve health services, create businesses, increase housing opportunities, bridge the digital divide, and strengthen the capacity of community organizations. VISTA members leverage human, financial, and material resources to increase the capacity of low-income communities across the country to solve their own problems. When VISTA members complete their service, they leave behind lasting solutions to some of our country's toughest problems. Volunteers In Service To America (VISTA), the AmeriCorps program fighting poverty, provides full-time volunteer members to nonprofit, faith-based and other community organizations, and public agencies to create and expand programs that ultimately bring low-income individuals and communities out of poverty. Objective The purpose of this requirement is to find current contact information for approximately 175,000 VISTA alumni. CNCS database of VISTA Alumni is incomplete and out-of-date. Currently, VISTA has little to no contact with alumni. This requirement will enable VISTA to build an alumni program as part of its overall outreach and recruitment strategies. This will also result in a mailing list for the upcoming 45th and 50th anniversaries of the VISTA program. Scope of Work VISTA is seeking a contractor to find contact information for alumni. Task 1: Finding current contact information • VISTA will provide the contractor with an encrypted DVD copy of the VISTA alumni database, which is a Micro Soft Access file. • Using only public records, a contractor will find the current home mailing addresses of as many of alumni as possible, based on their social security number and name in VISTA's electronic database. • The contractor will use only public records and databases to retrieve contact information for VISTA alumni. • The contractor will deliver the updated contact information as an Access file that can be uploaded into our existing database. • The contractor will also let VISTA know which individuals are deceased by flagging them in the Access file so that VISTA will not inadvertently contact a deceased individual's family. • The DVD copy of the VISTA alumni database will be returned to VISTA and all personally identifiable information in the possession of the contractor must be destroyed-proof of this destruction must be provided to VISTA. Deliverables and Schedule This project shall be completed by June 19, 2009. The contractor shall complete and deliver the following: • Post-Award conference to discuss the execution of the Purchase Order and Task within • Maintain weekly meetings with Contracting Officer's Technical Representative throughout the project and acquire VISTA's approval before beginning each task • Deliver an uploadable, Microsoft Access file containing current addresses of all VISTA alumni that can be found. • Return the DVD copy of the VISTA alumni database and certify that all other personally identifiable information has been destroyed. Corporation Furnished Property (Facilities, Equipment or Services) VISTA will provide alumni names and Social Security Numbers (SSNs) in a Microsoft Access file on an encrypted DVD. Note: All work shall be performed at the Contractor-Site. Corporation Commitment to Response Time The COTR and the contractor will determine an appropriate response time based on the specific tasks and deliverables (using the standard 10 business days as a reference point and not to exceed the 10-day reference point). Contractor Tasks/Responsibilities/Capabilities Contractor is responsible for assisting VISTA in preparing documents to secure Office of Management and Budget clearance (if necessary) for this project. Security Requirements Contractor must keep all alumni personal information secured, according to both FISMA and Privacy Act policies outlined below: As a federal agency, the Corporation for National and Community Service (CNCS) is subject to and complies with the security requirements of the Federal Information Security and Management Act (FISMA). The Contractor shall ensure that services and products provided under a contract resulting from this solicitation shall comply with the Corporation's information security program and privacy program policies, and Contractor Security Requirements available at http://www.nationalservice.gov/home/security_and_privacy_policy/index.asp. Contractor background screening requirements are included in the Security and Privacy requirements link above. In summary, contractors are required to complete the applicable background investigation request forms provided by the Corporation within 7 days after the individual's start date on the contract. If a contractor employee already holds a credential issued by a federal agency after background screening through OPM, the contractor may provide documentation supporting this status to the COTR. CNCS security officials will determine if the existing clearance can be accepted without further investigation.
