Description

The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for a Licensed Clinical Psychiatrist to provide services at the Pine Ridge PHS, IHS Hospital in Pine Ridge, South Dakota. The contractor shall work five (5) days per month, 8 hours per day, totaling 40 hours per week. The period of performance will be from April 13, 2009 through April 13, 2010 or 480 hours; whichever comes first. The Contractor shall perform according to the Statement of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-09-054, shall be in writing either by email or fax. Please provide the postal mailing address, email address and preference as to how the RFQ should be mailed. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be March 16, 2009. The closing date of the RFQ package is March 31, 2009.
