Description

The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for a Licensed Social Worker to provide mental health services at the White Earth PHS, Health Center in Ogema, Minnesota. The period of performance will be Date of Award through September 30, 2009; or 974 hours whichever comes first. The contractor shall perform according to the statement of work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-09-055, shall be in writing either by email or fax. Please provide the postal mailing address, email address and preferance as to how to mail the RFQ package. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be March 13,2009. The closing date of the RFQ package is March 30, 2009.
