This is a Transportation HCR (Highway Contract Route) Solicitation. The following information will be needed to complete forms that will be returned with your proposal: HCR # 638A2 Solicitation # 800-52-09 Date of Solicitation - 02/18/2009 Closing Date of Solicitation – 03/19/2009 Begin Contract Date - 4/13/2009 End Contract Date - 6/30/2012 For Mail Service Between Portageville, MO – Whiteoak, MO The Bid Submission Package will need the above information and be fully completed by the contractor. Send completed forms to the following address to be received by the Closing Date of Solicitation: WESTERN AREA OFFICE SOLICITATION NO. 800-52-09 1745 STOUT ST # 500 DENVER CO 80299-6200 BE SURE TO INCLUDE SOLICITATION NUMBER IN THE ADDRESS. All documentation for proposal must be received by the above address no later than Closing Date Of Solicitation by 1500 pm. Provide a photocopy of your Social Security Card or IRS Form CP 575 (EIN) *REQUIRED* NO SOLICITATION PACKAGES WILL BE SENT FROM THE CONTRACTING FACILITY OR THE AREA OFFICE. ALL DOCUMENTATION MUST BE DOWN LOADED AND PRINTED FROM THE ATTACHMENTS TO THIS SOLICITATION. NO PAPER COPIES WILL BE MAILED.
