Description

This is a Contract Delivery Service (Box Delivery) Solicitation. The following information will be needed to complete forms that will be returned with your proposal; HCR # 26370 Solicitation # 150-53-09 Date of Solicitation - 02/09/2009 Begin Contract Date - 4/1/2009 End Contract Date - 3/31/2014 For Mail Service Between City, St – West Union, WV City, St – Auburn, WV Attachment labeled " Forms Contractor must Complete and Return" will need the above information and be fully completed by the contractor. Completed forms will need to be sent to the following address; EASTERN AREA OFFICE SOLICITATION NO. 150-53-09 5315 CAMPBELLS RUN RD PITTSBURGH PA 15277-7070 Contractor must make sure to include the Solicitation number in the address. All documentation for proposal must be received by the above address no later than March 11, 2009 by 1500 pm. Provide a photocopy of your Social Security Card or IRS Form CP 575 (EIN) *REQUIRED* NO SOLICITATION PACKAGES WILL BE SENT FROM THE CONTRACTING FACILITY OR THE AREA OFFICE. ALL DOCUMENTATION MUST BE DOWN LOADED AND PRINTED FROM THE ATTACHMENT TO THIS SOLICITATION. NO PAPER COPIES WILL BE MAILED.
