Description
The Great Lakes Acquisition Center, VISN 12 VA, intends to negotiate a sole source contract with The GREEN HOUSE Project, Community Solutions Group, LLC, an affiliate of NCB Capital Impact, Inc. located at 2011 Crystal Drive, Suite 800, Arlington, VA 22202. To the best of our knowledge, Community Solutions Group, LLC is the only company with suitable past performance that will be able to provide expert technical advisory services to support the development, design, construction, and operation of new small home concept nursing home units for long-term care residents in Milwaukee and Tomah, Wisconsin and North Chicago, Illinois, as well as, education and training of staff. The small home concept is an innovative approach designed to address and improve the quality of care and life to residents by fostering a complete cultural transformation. The authority permitting other than full and open competition for this procurement is 41. U.S.C. 253 (c) 1, as implemented by FAR 6.302-1, only one responsible source and no other supplies or services will satisfy agency requirements. This authority is cited and justification made based on the determination that the Government's minimum needs can best be satisfied by services available from one source. The NAICS code for this requirement is 541618. This notice of intent is not a request for competitive proposals. No solicitation documents are available and telephone requests will not be honored. However, if a firm believes it is capable to perform these services, they must provide the minimum following information: 1. Demonstrated success in assembling an appropriate and comprehensive development and operations team. The company should have in existence small concept nursing homes with the universal worker staffing model and a philosophy of resident centered care. The small homes should have met all applicable regulatory standards, as well as, the Long Term Care Joint Commission Standards. The small homes should have been in operation at least six months and successfully reflect all aspects of the philosophy of cultural transformation. 2. Evidence of previous successful performance in the area of project feasibility, including operational analysis, market analysis, real estate development, and financing analysis. 3.Expertise in Development and Operations (a) Company shall show evidence of demonstrated competence in providing technical advisement in the area of architectural guidelines including building layout, design training, design review and approval. (b) Company shall show evidence of well developed Policy and Procedure guidance and technical assistance, which reflect a philosophy and understanding of cultural transformation. (c) Company shall provide staff training and be able to demonstrate that they have had experience in successfully transforming the culture of staff. (d)Company shall provide and have demonstrated experience for "move in" planning. (e)Company shall provide on site leadership training, implementation training, clinic support team training, universal worker on site training sessions with curriculum and "train the trainer" component, and post a post operational consulting trip. (f) Company shall provide a financial feasibility tool. This information must be submitted to the Contracting Officer by 4:30PM Central Time, February 11, 2009. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements and include a list of similar projects that have been performed within the past five years. Information shall be sent to the Contracting Officer, GLAC, 5000 W. National Avenue Bldg 5, Milwaukee, WI 53295 or via fax to (414) 902-5440. Responses received will be evaluated; however, a determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the GLAC will proceed with the sole source negotiation with The GREEN HOUSE Project, Community Solutions Group, LLC, an affiliate of NCB Capital Impact, Inc.
