- REQUIREMENTS: The Louis Stokes Cleveland Veterans Affairs Medical Center (CVAMC), 10000 Brecksville Road, Brecksville, Ohio 44141, intends to enter into a sole source contract with the American Red Cross, 3747 Euclid Avenue, Cleveland, Ohio 44115 for the following Blood and Blood Products services: WB 00160; RC 04210; LRBC 04730; LIBM 04800, FDRC 06400; CRYO 10100; PLTC 12000; LR PLTC 12070; PPHR 12011; PPHR (LR12710/12750; FFP 18201;ABFF; AUTP; CMVN; RCOP;This procurement is being conducted in accordance of FAR 6.302-1, only one responsible source and no other supplies or services will satisfy the agency requirements. The North American Industry Classification System (NAICS) 621991, size standard/$6.5 million, for Blood Banks. This Notice of Intent is not a request for competitive proposals. No solicitation documents are available and telephone and faxed requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products and services, as well as references from other customers who are using these products and services, to the Contract Specialist within five (5) calendar days from the date of publication of this synopsis, December 25, 2008 at 12:00 PM ET. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Offerors shall provide pricing for the blood bank services as described in requirements above. Quotes must fully satisfy the salient characteristics requirements listed herein. The total price offered shall include all charges (e.g., to include but not limited to delivery, shipping/handling, and pick up). All offers are subject to the provisions and clauses herein. Offerors shall complete FAR 52.212-3 Offerors Representations and Certifications-Commercial Items available for online registration at http://orca.bpn.gov or include a written copy with the quote submitted. Incorporate provisions and clauses in effect through Federal Acquisition Circular 2005-26 located at http://www.acqnet.gov/far/fac.html. The preceding Federal Acquisition Regulations (FAR) clauses and provisions may be accessed on the Internet at http://www.arnet.gov/far. The preceding Veterans Administration (VAAR) clauses and provisions may be accessed on the Internet at http://www1.va.gov/oamm/oa/ars/policyreg/vaar/. All information must be sent to the Contract Specialist, Jean Mays via email: Jeanette.Mays@med.va.gov. Responses received will be evaluated however; a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the Cleveland VAMC will proceed with the sole source negotiation with the American Red Cross on December 30, 2008.
