- CLINICAL LONG TERM CARE MEDICAL DIRECTOR STATEMENT OF WORK This position at VAMC, Manchester, NH is a 40 hour/week position expected to last at least 90 days. Position comes under the Chief of Staff and reports to the Local Service Line Manager., Geriatrics & Extended Care and the director of the Manchester VA Medical Center. Contractor directs and coordinates admissions to medical services in the Community Living Center(CLC) ensuring that medical care provided is adequate and appropriate. Serves as member of medical staff, attends meetings and helps to ensure adherence to bylaws, rules and regulations As a member of the medical staff, the Medical Director for CLC, is ultimately accountable to the Medical Center Director. Contractor assures that the CLC medical staff functions and medical operations are managed and consistent with applicable standards and regulations and are follow Joint Commission accreditation requirements. Contractor directs and coordinates medical care for veteran residents in the CLC program. Contractor makes credentialing and privileging recommendations to the Executive Committee of the Medical Staff. Contractor communicates medical staff responsibility and medical care policies, procedures and guideline to all licensed independent practitioners providing and ordering care within the CLC services. Contractor must provide clinical oversight of providers rendering geriatric services to veteran residents to ensure they are meeting the standard of practice. Contractor serves as Medical Director to the HBPC program Contractor serves appropriate staff physician's availability by assisting in arranging coverage. Contractor participates in establishing policies, procedures and guidelines to ensure comprehensive services. Contractor helps develop emergency treatment procedures for residents. Contractor helps develop resident transfer procedures and establish policies to ensure all health care professionsls practice within scope of their licenses. Contractor establishes mechanisms for appropriate consultation when necessary expertise is not available w/I GEC Care Line Contractor is responsible for day tp day clinical operations in CLC skilled care unit. Person will participate in developing and maintaining adequate medical records systems and documentation requirements. Interfaces with all services comprising the interdisciplinary team. Consults with the CLC pharmacist and conducts periodic overall reviews for appropriate use of pharmaceutical products minimizing of polypharmacy. Conducts interdisciplinary rounds and other practitioners, nurses, pharmacists and nutritionists in CLC. Develops protocolto assure wide variety of patients can be admitted to CLC. Collaborates with GEM LSLM on matters pertinent to maintenance of the CLC's ability to meet the veterans residents needs. Collaborates with the LSLM to ensure the adequacy and appropriateness of the CLC's scope of services, medical equipment and medical support staff.. Contractor explores potential opportunities for and supports the GEC LSLM with development of future veteran-resident care programs and systems. Contractor participates in managing the environment by identifying hazards to health asnd safety by making recommendations to the GEC LSLM. Contractor participates in performance improvement initiatives for GEC medical services as an integral part of improving organizational improvements. Contractor participates in educational programs of this facility and maintains compliance with continuing education requirements. Also serves as an educator and resource on geriatric medicine w/I the GEC care line. Contractor assures clinical aspects of regulatory standards are met and maintained including, but not limited to TJC,OSHA,CAP, and others that apply. Contractro covers for the GEC LSLM when needed to ensure representations of committees such as QEB,GEC Management Board, ECMS, Supervisory Forums, JC Readiness, ect.
