Amendment to Request for Proposal VA-2453-RP-0077: 2) The Statement of Work has been revised and has been attached below. 3) The services of a Certified Medical Physicist are required to perform the work. Of Note: The due date for receipt of proposals remains January 15, 2009 @ 3:00 P.M E.S.T. Acknowledgement of receipt of this amendment is required by completing Acknowledgement section of Page 5 of the RFP. The following three pages are to replace pages 8, 9 and 10 of the RFP: VA-243-RP-0077 STATEMENT OF WORK Contracting Officers Technical Representative (COTR) and Contact Point: Mr. Salvatore Tatta, Chief, Biomedical Engineer, is the COTR for this project. He can be contacted at (718) 584-9000 ext 6870. Normal Business Hours: Monday - Friday, 8:00 am - 4:30 pm, excluding Federal Holidays. Location: James J. Peters VAMC 130 West Kingsbridge Road Bronx, New York 10468 Radiation Therapy, GD area Equipment: Varian model Clinac 2100 iX Silhouette 6/15MV & 5e configuration Medical Linear Accelerator with all purchased accessories, including but not limited to: a. Dual photon & multiple electron Energies b. Multileaf Collimator c. Digital Portal Imager (Portal Vision) d. 4D Treatment Integrated Console (TIC) workstation e. Patient Positioning Lasers f. Eclipse IMX 3D Integrated Treatment Planning System g. Portal Dosimetry Software h. Respiratory Gating System i. VARiS Record and Verify System Installer: Varian Medical Systems (Original Equipment Manufacturer) Installation: The Equipment will be installed at this Location by the Installer approximately on December 16th, 2008 over an estimated thirty (30) consecutive business day period beginning no sooner than January 15, 2009. This is a tentative start date which is SUBJECT TO CHANGE. Task: The Contractor shall supply all Travel, Materials, Test Equipment and Services necessary to perform all phases of Acceptance Testing on the Equipment and Certify/Commission it for Clinical Use. General Requirements: The contractor shall: a. Review/complete the on-line VAMC Contract Worker Orientation Handbook and abide by all facility rules/regulations. b.Comply with all OSHA requirements as related to Performance of Work. c.Obtain Contractor Photo Identification Badges from VAMC Police Dept. d.Furnish, Wear and Maintain Film Badges. Room Radiation Shield Assessment (Final): 1. The contractor shall perform a Final Safety Survey on the surrounding areas adjacent to and above the Location and shall measure/record Radiation Leakage after the Installer has completed all beam tuning/ calibration/adjustments. 2. The contractor shall determine/confirm Shielding Adequacy based on Radiation Leakage measurements and on patient load/treatment factors. Acceptance Testing/Commissioning: All requirements listed under New York State Department of Health Guidelines for Linear Accelerator Equipment Testing and all parameters necessary for acceptance testing and Commissioning this Equipment for Clinical Use shall be performed/tested/verified/beam data collected and loaded in accordance with American Association of Physicists in Medicine recommendations including but not limited to: a.Mechanical/Functional testing of: Isocenter, couch, gantry, collimators, position indicators & lasers, pointer, trays, wedges, table clamps, table accessory rails, Portal Vision robotic arm, and control pendants. b.Physics testing of: All photon beams, all electron beams, flatness & symmetry of beams, beam & light field congruence/coincidence, dose rate accuracy, reproducibility of dose, field size factors, constancy vs. gantry angle, wedge factors, and tray transmission factors. c.Commissioning of: Varian Clinac 2100 iX Silhouette with all accessories listed above in Equipment. Timeframe: 1. After the Installation is complete, the contractor shall work during Normal Business Hours and shall have thirty (30) consecutive days to complete all phases of Acceptance Testing/Commissioning in the following order including: a. Final Room Radiation Shield Assessment; b. Acceptance Testing; c. Commissioning of all Treatment Plans (with Eclipse); d. Commissioning of Record and Verify system (VARiS); 2. All work performed outside of Normal Business Hours shall be coordinated/approved in advance with the COTR. 3. Exact start times/dates are: a. SUBJECT TO CHANGE. b. Dependent on the progress of the Installation. c. Shall be coordinated with the COTR. Experience: The contractor shall be an Independent/Certified/Licensed Medical Physicist with no direct affiliation with or interest in Varian Medical or its subcontractors. The contractor shall have an insured, active, legitimate business in this field and shall demonstrate practical experience working with this Equipment by providing the James J. Peters VAMC with a minimum of three (3) references. Test Equipment: The contractor shall provide and utilize only fully calibrated test equipment / measurement instruments to perform the Task and shall furnish copies of calibration certificates. Liquidated Damages: 1. Failure to complete the Task in the allotted Timeframe will result in liquidated damages being charged to the contractor at a cost of $4,000.00 per day for every day the contractor is past the allotted completion time of thirty (30) consecutive days to complete all phases of Acceptance Testing/Commissioning. 2. The Timeframe clock will pause when: a. The contractor identifies an out of specification parameter which requires recalibration to correct. b. The Equipment is inoperable and requires service or parts to repair. It will resume after repairs/corrections have been completed by the Installer. Documentation: 1. The contractor shall provide the COTR with written confirmation of Shielding Adequacy upon completion of the Final Room Radiation Shield Assessment. 2. The contractor shall provide the COTR with a Full/Detailed/ Complete/Written Acceptance Testing Report within five (5) business days after completing the Task. Justification: The James J. Peters VAMC purchased a New Varian model Clinac 2100 iX Silhouette Medical Linear Accelerator with 6/15MV & 5e configuration. This equipment provides Radiation Therapy for Oncology Patients. It is utilized by specialized trained staff to administer a predetermined course of radiation treatment doses which subsequently destroy cancer cells/tumors. An injurious or lethal radiation dose potentially may be administered to the patient if this equipment is not in full compliance with New York State Department of Health Regulations or not calibrated to Original Manufacturer's Specifications. Quality Assurance/Acceptance Testing by a Certified/Qualified Medical Physicist is needed to Verify Room Shielding/Radiation Output/Patient Safety/Equipment Performance and is necessary to Commission this Equipment for Clinical Use.
