- Treasure Island Job Corps Center is accepting bids for On-Site Dental services.Treasure Island Job Corps Center, a federally-funded residential training facility servicing 600 resident and non-resident trainees ages 16 to 24 years old, would like to invite qualified bidders to provide on-site dental health services at Treasure Island Job Corps Center located at 655 H Avenue, Building 442, Harvey Milk Admin Bldg, San Francisco CA 94130. The successful bidder will provide this service for a period of 2 years and option to extend for another 3 years, renewable every year. This option is at the Center's discretion to exercise. All interested parties will submit a pricing program and proposal with a written response to Marizen Tomas,.Purchasing Supervisor via email: tomas.marizen@jobscorps.org) or by fax, 415-705-1839, or by USPS or other acceptable delivery methods, by the close of business day, 12/29/2008. The successful bidder must comply with all Federal Acquisition Regulation clauses and be compliant under all related federal statutory requirements and regulations. Interested parties are to determine their offer(s) based upon the specifications: Specifications: Dental (Oral Health) services will be provided for *Twenty Four (24) hours per week for the Center Dentist and up to Twenty-four(24) hours per week for the Dental Hygienist.* The services will be provided at the Treasure Island Job Corps Center facilities located at 655 H Avenue, Bldg 442. San Francisco CA. Center staff will provide outside clinical services only in emergency situations. *Work hours indicated for each of the above services are subject to change upon the discretion of the Center Director. The period of performance for this agreement is from Feb 1, 2009 through Jan 31, 2011 with three (3) option years extending to Jan 31, 2014, renewable every year. Option years are to be exercised upon management discretion.
