- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 12.6, with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-27. This solicitation is a 100% set aside. The NAICS code is 622310, with a size standard of $34.5 million. The delivery, acceptance, and FOB point is the contractor's facility in Reno, NV. The contract will include a one year basic period and two one-year options. Only electronic offers will be accepted. Offers are due to joel.burgess@va.gov by NLT 1:30 p.m. on November 12, 2008. The Description of Work and Price Schedule: 1.Scope of Work: The contractor shall provide use of an Outpatient Surgical Center to perform the surgical procedures listed below, to the VA Sierra Nevada Health Care System (VASNHCS). The facilities will include the operating room and recovery room staff. Procedures will be performed by VASNHCS Surgeons credentialed at the facility who will evaluate patients pre-operatively with a detailed assessment to determine the need for biopsy or further procedures. The VASNHCS will see patients in follow-up after the surgery and manage their further care. All specimens will be transported by the VASNHCS surgeon and processed and reviewed by the pathology department at VASNHCS. Arrangements for radiological wire guided imaging will be made separately and are not covered by this contract. Anesthesia services will be billed directly to VASNHCS. The contractor will not assume or incur any liability for the anesthesia billing. 2. Evaluation of patient with written exam results: 2.1. Consult will be entered by the requesting VA surgeon and approved by the Chief of Surgery or designee. Facility fees, anesthesia and radiological costs will be billed to VASNHCS. 2.2. The VASNHCS will document all care provided at the Surgical Center in VASNHCS CPRS system. All VASNHCS physicians must be privileged at the Surgical Center under the auspice that their services there are considered an extension of the VA. There is no remuneration directly to the VASNHCS physician for these services. 2.3. Medical malpractice for the VASNHCS physician is covered under the VA arrangement. 2.4. Contractor Surgical Center will provide: 2.4.1. Facility space (includes pre and post-op nursing care, check-in, operative room use). 2.4.2. Equipment required to perform the listed services. 2.4.3. Anesthesia Service: Provided by current anesthesia service utilized by Contractor Surgical Center. 2.4.4. Radiology Services: Contractor Surgical Center must be located adjacent to a resource which will provide imaging equipment necessary to do wire guided procedures. 2.4.5. These procedures are very time sensitive and the contractor must be accommodating with scheduling. 2.5. In order to adequately protect VA patients, neither the Contractor Surgical Center nor VASNHCS physician shall introduce new procedures or services without prior recommendation to, and approval from, the VASNHCS Chief of Staff/Clinical Medical Director. 3. Quality Assurance 3.1 Contractor Surgical Center and VASNHCS shall perform services under this contract in accordance with the ethical, professional, and technical standards of the healthcare industry, and must meet, or exceed, the current quality assurance standards established by the American Medical Association, Joint Commission, Department of Veterans Affairs regulations and medical staff by-laws and those of other professional associations, which specify standards of performance for the medical profession. 3.2. Contractor will maintain and provide evidence of the appropriate accreditation standards applicable to Surgery Centers and consistent with community standard protocols. Accreditation will be maintained for the life of the contract. Contractor will notify the VASNHCS immediately of any sanctions or loss of accreditation privileges. 3.3 Operative Notes submitted by Contractor to VASNHCS will contain at a minimum: Preoperative Diagnosis, Postoperative Diagnosis, Procedure Performed, Operating Surgeon, Anesthesiologist, Indications, Description of Procedure, Complications, Proper time out procedures to insure correct site surgery, Sponge, Instrument, and Needle Counts, Disposition of the patient, Findings Performance Period: Base Period is November 14, 2008 through October 31, 2009.
