- Description
- The Department of Veterans Affairs Network 22 Logistics office has a requirement for Skilled Home Care Services by full services Home Care Agencies to provide services for Veterans in the following counties: NV - Nye, Lincoln, Clark, Esmeralda; Utah - Washington County; Arizona - Mohave County. The Skilled Home Care Services and Therapies include, but are not limited to; 1.) Skilled Nursing, 2.) Physical Therapy, 3.) Speech Therapy, 4.) Occupational Therapy, and 5.) Medical Social Services, provided by licensed health care professionals as ordered by the medical staff of the VA. Home Care Agency services may include but are not limited to: admitting veterans; assessing veterans (including initial and ongoing assessments); care planning (including the care planning process); coordinating, supervising, and evaluating the care services provided; scheduling visits or hours; and discharge planning. Skilled Home Care Services include but are not limited to: Nursing services, treatment, and diagnostic and preventative procedures requiring substantial specialized skill; Veteran safety monitoring; Medication administration, including intravenous fluids, medicines, pain management, TPN, chemotherapy, and necessary care of the delivery lines and equipment; Required lab work with reports to the physician/provider; Veteran education and follow-up; Teaching, supervision, and counseling veteran and family members regarding nursing care and other related Veteran problems; Rehabilitation regimen; Evaluation and regular re-evaluation of Veteran home care needs; make appropriate recommendations; Development, implementation, oversight, and modification of the Veteran plan of care; Documentation of the Veteran's care and progress in the clinical record, including a discharge summary within 72 hours of discharge; Physical assessment and re-assessment per written policy; and Documentation of outcomes of care/services provided. B. Bowel and Bladder care services, if ordered, shall be performed by a Registered Nurse (RN) or Licensed Practical Nurse (LPN). The home care agency must be certified by Medicare and/or Medicaid and meet the requirements for the Joint Commission on Accreditation of Healthcare Organization (JC) standards. It's the VA's intention to solicit proposals on a competitive basis of Firm-Fixed Price, Indefinite Delivery, and Indefinite Quantity (IDIQ) contract. The contract will include a base year (one year from date of award), with four (4) one-year renewal options that can be exercised at the option of the Government. This solicitation will be issued as a commercial item in accordance with FAR Part 12 and shall be evaluated in accordance with FAR Part 15. NAICS is 623110, Size Standard $12.5 Million.
