- The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 3), VA Medical Center, Brooklyn, New York, is currently seeking firms, Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, Hubzone, 8(A) or Small Business with the ability to provide preventive maintenance and inspection as well as repair to 2 Planmeca Dental X-Ray Machines as belowFirms desiring consideration shall email directly to the Contracting Officer referencing the following in the subject field, Planmeca Dental X-Ray Machines, then contain the following information within the body of the e-mail name of firm, Socio-Economic category of the firm (SDVOSB,VOSB, 8A, HUBZONE or SMALL BUSINESS), complete address, phone & fax number of contact. All emails from interested parties must be received by 9:00 AM, September 25th 2008. Email address as follows, david.shemtob@va.gov or by fax (718) 741-4722. Contracting office is located at the James J. Peters Medical Center, 130 West Kingsbridge Road, Bronx, NY 10468. Service and Maintenance contract for 2 Planmeca machines in Dental area of VAMC Brooklyn Campus. Coverage 10/1/2208-9/30/2009. Items: Promax and PM-2002 CC This is a full service contract to include all necessary labor, tools, equipment, transportation and parts needed to perform repair and maintenance on above listed equipment. All equipment shall be maintained in proper operating condition as specified by the manufacturer. Vendor must have access to original parts, manuals, and schematics to perform the service. Hours of Coverage and Response Time: Mon-Fri 8:00AM-4:30 PM excluding Federal Holidays. Contractor must respond via a phone call within 8 hours of a service request and have an on site response within 24 hours of initial request for service. Qualifications of contractor: All repairs and preventive maintenance shall be preformed by manufacturer trained technician with the ability to demonstrate related experience. No allowance will be made for contractor not having qualified personnel available to respond during the specified times in this contract.
