- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. VA-259-08-RQ-0313 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-26. This acquisition is unrestricted under NAICS code 334510. The requirements in this solicitation: Spyglass Visualization System, as manufactured and distributed by Boston Scientific, 1 package, consisting of the following components: Camera and Camera Head Cable, P/N M00546100, 1 Each; Video Monitor, P/N M00546120, 1 Each; Travel Cart, P/N M00546160, 1 Each; Light Source, P/N M00546190, 1 Each; Light Cable, P/N M00546210, 1 Each; Ocular, P/N M00546040, 1 Each; 120V Isolation Transformer, P/N M00546260, 1 Each; Irrigation Pump with Foot Switch, P/N M00546140, 1 Each; Probe Storage Tray, P/N M 00546060, 1 Each; Power Cable Pack, P/N M00546250, 1 Each; Direct Visualization Probe, P/N M00546030, 2 Each; Access and Delivery Catheter, P/N M00546230, 10 Each; Forceps, P/N M00546270, 10 Each; Spyglass System Operator Manual P/N M00546180. All quotes submitted in response to this RFQ must remain firm through October 1, 2008. Delivery required 30 days after receipt of order. Shipping shall be FOB Destination to: VA Eastern Colorado Health Care System - Warehouse, 1055 Clermont Street, Denver, Colorado 80220.
