Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Request for Quote is W9124B-08-T-01111 issued as request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16 dated 22 March 07. The associated NAICS code is 813110. Contractor shall provide the following specific tasks, ITEM DESCRIPTION AND QUANTITY FOR FIRM FIXED PRICE (FFP): General. The Center Chapel Musician plays the piano or electronic keyboard to enhance worship during religious services. The pianist may or may not accompany a choir or praise team. The Director of the team will be the leader of the music. Scope of Work for CATHOLIC MASS SERVICE PIANIST. The Contractor mutually agrees to perform one or more of the following services and be paid the applicable rate for the services after satisfactory rendition. It is further agreed that the Contractor will perform to the best of his/her ability. In a practical sense, the contractor will accommodate the music and style of performance to the requirements of the religious service. The Contractor will be able to play requested music appropriate to the service either by sight reading or by ear with little or no rehearsal. The music must remain appropriate to the style and tradition for which the religious service is provided. The Contractor will obtain guidance for the appropriate music from the senior chaplain responsible for the religious service or his designated representative. The Contractor will either personally perform the services described in this Statement of Work (SOW) or will sub-contract them to a substitute of equal or greater proficiency who has also been approved by the Installation Chaplain.. This Contract is payable only to the Contractor. The Contractor will be paid only for the services performed. In the event the Contractor employs or sub-contracts this service to another individual, the Contractor will bear the responsibility for payment for that portion supplied by the other individual. Failure to provide the service, either personally or through the use of another qualified party may result in termination of this contract. The failure of a second party to provide for the Contractors contracted services will be viewed as failure of the Contractor to provide the required service. The standard for compliance is punctual commencement of required services, competent performance, and choral blend with consideration for the traditions of the service. Contractor will be paid for a unit of service if contractor arrives at the scheduled time and no performance is required of the contractor. Contractor will not be paid for services and rehearsals that are officially cancelled by the sponsoring Chaplain prior to the service. The contractor will have access to the chapel facilities of the installation which are suitable for performing the service, as determined by the Installation Chaplain. Contractor duties will normally be performed during evening and weekend hours. The contractor may be required to provide services on legal federal, state, and local holidays.Contractor shall provide the following specific tasks: CLIN0001: Play for Catholic Mass Service on Sundays-This line item includes: Play for Catholic Service on Sundays: This service will occur at the Ft. Irwin Center Chapel. It will normally begin at 1630 hours conclude at the end of the service. The contractor will be present at the chapel at least 15 minutes prior to and 15 minutes after each service to provide mood music during that time as requested by the officiating chaplain. The Catholic chaplain or his designated military or DoD employee representative will provide service information to the pianist by the Tuesday prior to the planned religious service if the service requires a rehearsal or by Thursday if there is no rehearsal, QTY 52. CLIN 0002: Choir Rehearsals for Catholic services This line item includes: Provide accompaniment for Catholic Service and Catholic choir rehearsals: Contractor will coordinate with the choir and Catholic chaplain concerning the music planned for the upcoming service. Rehearsing with each choir for at least one hour during the week in preparation for the religious service. Additionally, rehearsing with the Catholic choir for one hour prior to Mass on Sunday, QTY 52. CLIN 0003: Special Religious Services This line item includes: Special Religious Services: Coordinate with the chaplain in charge and accompany for services as determined by the Installation Chaplain. The Contractor may be requested to provide appropriate music for Memorial Ceremonies, Memorial Services and Funerals. Additionally the Contractor may be requested to accompany at Memorial Day services, Holy Days (All Saints, Ash Wednesday, Good Friday, etc), Thanksgiving, and Christmas Eve, QTY 12. CLIN0004: Rehearsal for special Religious Services.This line item includes: Rehearsals for Special Religious Services: Coordinate with the choir and about rehearsal for services such as Memorial Day Services, Holy Days (All Saints, Ash Wednesday, Good Friday, etc), Thanksgiving, Christmas Eve, QTY 12. Qualifications. a. Contractor shall demonstrate competency and be able to play requested music appropriate to the service either by sight reading or by ear with little or no rehearsal. Contractor and/or the contractor's employee shall possess the ability to communicate effectively with chaplains, volunteers, and government representatives in English.b. The Contractor will engage in an interview and audition process held by the Installation Chaplain and the Senior Chaplain of each congregation to determine his/her qualifications and ability to work in the chapel services religious environment. c. Contractor and/or contractors employee shall have the ability to interact effectively with individuals from different religious groups. d. Background check: Before any contractor or any contractor employee can begin their duties, any person working in direct contact with children under the age of 18 must submit to a background check to ensure such individuals have had no allegations of improper behavior involving child abuse or drug use. Any negative results must be resolved before the contractor or contractors employees can begin to work The contractor or any contractor employee shall not conduct any performance or rehearsal that involves children under the age of 18 without line of sight supervision by an Installation Chaplains representative until satisfactory results of the background check have been received. e. The contractor shall supply all necessary information to complete the background check(s) to the Installation Chaplains Office for each person who performs under this contract. Appearance. The contractor shall maintain a well-groomed appearance appropriate to the services traditions as determined by the Catholic Chaplain. Support: Contractor shall be authorized use of government equipment such as a piano, hymnal, microphone, and sound system. A photocopy machine at the chapel will be made available for use by the Contractor on an as available basis to fulfill contract requirements. There will be no infringement or violation of copyright laws. Contractor Furnished Property. Contractor shall provide his/her own transportation and all necessary equipment to fulfill this contract except as stated elsewhere in this contract. Physical Security. The Contractor shall establish and implement methods to ensure the physical security of all U.S. Government property.Safety. The Contractor shall be aware of local safety requirements and ensure that all reasonable efforts are made to protect all persons from harm and U.S. Government property from damage.Restrictions. Government property made available under this contract is only for use in the performance of services described in this contract.Disclaimer. The government will not be responsible in any way for damage to the contractors supplies, materials, equipment, or to personal property as a result of fire, theft, accident, natural disaster, or other events.Liability. For the purpose of this contract, the Contractor is considered to be an independent contractor and not as an agent of the U.S. Government. During performance of services, the Contractor shall be liable and will indemnify and hold harmless the Government, its agents and employees against all actions or claims for damages to persons or property, including death arising from fault, negligence, wrongful act, or wrongful omission of the Contractor, his agents or employees utilized in the provision of services.Compensation. The contractor will be paid upon satisfactory completion of the services contained in this SOW and upon presentation of an invoice IAW terms of this contract. The Contractor is authorized partial payments on a monthly basis. By intent, this contract is a non-personal services contract and does not create an employer-employee relationship. There are no provisions for lodging or employee benefits of medical care, retirement, or workers compensation. The contractor is responsible for his/her own tax liabilities. Payments made under this contract are not subject to Federal Income Tax withholding. Requested Period of Performance: 01 Oct 08- 30 Sep 09.
