- This is a sources sought announcement only. This is not a solicitation announcement. The purpose is to gain knowledge of current market capabilities. The Department of Veterans Affairs (VISN9), is seeking potential qualified contractors to provide a Capitated Community Based Outpatient Clinic (CBOC) in or around Dyer County in Dyersburg, Tennessee. The CBOC Clinic will provide preventative and primary care as well as mental health services to approximately 1500 veterans in the surrounding Dyersburg TN area. The initial contract will be established for a one year period with four options years. The contractor shall furnish health care providers, medical facilities; equipment and supplies, emergency fill pharmaceutical prescriptions and administrative functions to fulfill the level of care and range of services necessary to meet the primary medical and mental health needs of enrolled patients. The contractor shall be responsible for the administration and management of all aspects of this contract and the health plan covered hereunder, including responsibility for all employees, agents, subcontractors and any other entity acting for or on behalf of the contractor. In addition, please indicate whether your company has experience with the Department of Veterans Affairs Capitated CBOC Program, and if it is your intention to be a joint venture with another company.
