- INVITATION TO BID HEALTH SERVICES Adams and Associates, Inc. contracting for the U.S. Department of Labor, for the operation of the Joliet Job Corps Center, a 280 student residential training facility located at 1101 Mills Road, Joliet, IL 60433 is currently soliciting written bids for Health Services. Attached please find the Scope of Services, that will be required under this Health Services Agreement. All written bids should be submitted in writing to the Purchasing Department by Monday, September 22, 2008 at 3:00 p.m. Bids may be faxed to 815-727-9457 Attn: Michael Fabry, Purchasing Department, emailed to: fabry.michael@jobcorps.org, or mailed to: Joliet Job Corps Center Attn: Michael Fabry 1101 Mills Road Joliet, IL 60433 This Agreement shall have an inital term of 2 years October 1, 2008 through September 30, 2010. In your respone, please provide pricing for Year 1, October 1, 2008 through September 30, 2009, and for year 2, October 1, 2009 through September 30, 2010. Also, please provide separate pricing for each potential optional year 3, 4 and 5. If you do not wish to quote please indicate and return to us. Adams and Associates, Inc. reserves the right to reject any bids and select the bid deemed most responsible. Bidders should be aware of bid requirements for federally funded projects and act accordingly. Joliet Job Corps Center is a government facility and entitled to GSA pricing. I.SCOPE OF SERVICES The health services to be provided under this subcontract shall conform, at a minimum, to the health services specified in the Job Corps Federal Regulations ( Title 20 Code of Federal Regulations, Part 638.510), in the Job Corps Policy and Requirements Handbook (PRH), Chapter 6 and applicable Technical Assistance Guide (TAG). To accomplish these ends, the Physician shall: A.Provide health services by a Physician a minimum of twelve (12) hours per week, Any additional hours must be approved in advance by the Center Director. The services will provided at the facilities located at the Center Wellness Services Clinic. Center staff will be provided clinical services in emergency situations only. The maximum weeks of service will not exceed 48 weeks per year. Services are not required during DOL-mandated Center shutdowns that occur during the summer (2 weeks in July) and winter (2 weeks in December). The Agency will not be paid during this period, unless the Physician is required by the Center to provide services or attend Center mandated training during this shutdown. The Center will not the Agency of planned shutdowns as early as practicable. B.Develop, implement, and monitor the total Center Health Program. Specific duties include but are not limited to: 1.Advising the Center Director and Wellness Services Staff on all medical and health related matters, including, but not limited to: staffing requirements, students medical terminations, including the furnishing of diagnosis, prognosis, and recommendations for future care, utilization and cost of services, personalization of patient care, and any other health related areas deemed appropriate by the Physician and/or Center Director. 2.Consulting with Center staff on ordering, accounting for and safeguarding health equipment, supplies and medications. 3.With due regard for Job Corps requirements, prioritizing student health needs and setting program direction accordingly. To accomplish this end, the Physician shall confer with the Center Wellness Services Manager ( or chief nurse or medic), the Center Dentist and the Center Mental Health Consultant as required. 4.Managing health care costs. To accomplish this end, the Physician shall meet with the Center Director or Center Finance and Administration Director on a quarterly basis to review expenditures for the prior period and establish restraints, if necessary, to stay within budgetary guidelines. The Physician may involve the Job Corps Regional Health Consultants in this process, if needed. 5.Developing a system to promote the coordination and integration of the health components (Medical, Dental, Mental Health, Environmental Health and Health Education) with other Center activities. This will be done in cooperation with the Wellness Services Manager, the Center Dentist, and the Mental Health Consultant. 6.Providing advice and assistance to the Health Education Instructors on the implementation of the Health Education Program. 7.Ensuring the maintenance, confidentiality and safeguarding of all student health records. 8.Assisting the Center Director to arrange for the required Center quarterly environmental health inspections by a qualified Environmental Health Specialist employed by a public Agency or privately employed. 9.Issuing and maintaining signed and dated standing orders for medical care in accordance with PRH Chapter 6 and Tag 6-Q. C.Provide direct medical services to students, including: 1.A cursory medical inspection for each student within the first 48 hours after arrival on Center and a definitive medical examination and health history of each student within 14 days of arrival on Center. The medical examination will include the interpretation of all routine laboratory tests, when necessary, X-rays. (Fees for consultation by a radiologist are not included in this agreement). Although much of this work may be delegated, the Agency bears the ultimate responsibility for the quality of work performed. 2.Diagnosis and treatment of illness and injury, including the Physician’s availability on Center for sick calls or appointments as scheduled by the Center Director or his/her representative. 3.Arrangements for preventative immunizations and detection and control of communicable diseases as specified in PRH Chapter 6.10, Section R1.d: Immunizations. 4.Provision of all necessary off Center referrals for specialists and institutional care with Center Director concurrence. 5.Ensuring that medical reports and vouchers are accurate, reasonable in amount and delivered to the Center in a timely manner.
