Description
This is a combined synopsis/solicitation for commercial items prepared inaccordance with the format in FAR, Subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only solicitation issued. STATEMENT OF WORK: Quotes are being requested and a written solicitation will not be issued. The solicitation number is VA-247-08-RQ-0326 and is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-26, NAICS is 325412. The Department of Veterans affairs Birmingham, AL requires the following; (1) Contractor shall provide all labor, supervision, and transportation necessary to provide daily deliveries to the Department of Veterans Affairs Medical Center, (VAMC) Nuclear Medicine (Hot Lab), 700 South 19th Street, Room 2340, Birmingham Alabama 35233. Thallium-201 pre-calibrated as of 12:00 noon on the day of delivery. The number of doses (ranging from 3.0 to 6.0 Millicuries per dose syringe and/or as requested for re-injection dose(s)/syringe one (1.0) to two (2.0) Millicuries as requested) are estimated. Deliveries shall be made between the hours of 7:00 AM and 4:30 PM, Monday through Friday (excluding weekends and Federal Holidays). The Contractor will provide daily deliveries as needed and/or requested by the Nuclear Medicine Personnel/Physician(s). The quantities for thallium are estimated and deliveries are included in unit dose cost. NO ADDITIONAL CHARGES WILL BE ACCEPTED FOR DELIVERIES AND THERE WILL BE NO LIMIT ON THE NUMBER OF DELIVERIES MADE TO THE VA MEDICAL CENTER NUCLEAR MEDICINE SERVICE. (2) All radiopharmaceutical orders must be within +/- 10 percent (10%) of the doctors order allowing only for correcting calibration date and time. (3) All radiopharmaceutical orders must be in total compliance with the State of Alabama and/or Federal Regulations (Nuclear Regulatory Commission (NRC), FDA, DOT, OSHA, and EPA, etc.) regarding radioactive materials. (4) All shipping containers must be free of radioactive contamination both inside and outside container(s). All containers shall be in compliance with State, Federal agencies, (i.e. department of Transportation (DOT) and NRC). (5) Contractor must supply with each shipment/order/delivery a system for spent (used) syringes that meets OSHAs Bio-Hazard Policy/Regulation(s) and is needless. (6) No additional delivery charges (fuel surcharges) will be accepted, and there will be no limit on the number of deliveries made to the Nuclear Medicine Service. (7) Invoice shall be paid monthly in arrears upon receipt of statement/properly submitted invoice shall include the description and purchase order number. (8) Contractor shall Notify designated personnel (VA-Nuclear Medicine/Fiscal service) of any problems that arise regarding billing/unpaid invoice within (60) days of order date. If contractor fails to notify VA of problems, unpaid monies shall be forfeited. Contractor shall bill appropriately and timely and notify VA if problems arise with the billing issues. (9) If contractor cannot supply dose, due to technical problems, etc., contractor is required to buy the dose from another source and transport to VA so that our Veterans patient care is not compromised.(BVAMC Radiation Safety Officer must be contracted before purchased dose is sent to ensure regulatory compliance (afterhours pager 205-917-0694). (10) EMERGENCY DELIVERIES: In case of an emergency, the contractor shall be required to make deliveries within on (1) hour, (excluding weekends and Federal Holidays as requested by the Department of Veterans Affairs (DVA) Nuclear Medicine Service Personnel. (11) LICENSE: A copy of the DVA medical Centers Radioactive Material Permit will be furnished to the Contractor upon award. (12) ADDITION/REDUCTION/CANCELLATION: The DVA Medical Center will have the option to add, reduce or cancel an order(s) by giving the Contractor at least one (1) hour notice. The SCHEDULE OF PRICES FOR THE BASE YEAR AND FOUR (1) Year OPTION BEGINNING August 1, 2008 THROUGH July 31, 2009 SHALL BE LISTED AND PRICED AS FOLLOWS: Estimated Doses 500 ea. Thallium-201 Doses $_______ ea. $_________ TOTAL. Submit offers on Company letterhead stationary in accordance with FAR 52.212-1 Instructions to Offerors commercial items. Acceptance time will be 60 days from offer due date.
