Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The Department of Veterans Affairs, VA Medical Center Grand Junction located in Grand Junction, Colorado is performing market research to determine if there is a sufficient number of qualified (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small or Emerging Small Business to set aside a base plus four option contract for LABORATORY SERVICES FOR ANATOMIC PATHOLOGY. The Government will use responses to this notice to make the appropriate acquisition decision. The proposed solicitation will be issued in accordance with FAR Parts 12 and 13. LABORATORY SERVICES FOR ANATOMIC PATHOLOGY TO BE PROVIDED AT THE VETERANS AFFAIRS MEDICAL CENTER GRAND JUNCTION, COLORADO Provide both, on-site and at contractors site, conventional services for anatomic pathology and clinical laboratory daily Monday through Friday. Must provide 24-hour coverage 365 days per year for emergent procedures such as frozen sections, autopsies, etc. PROCEDURE: Grossing & reading of surgical specimens Perform at Contractor's Lab Reading cytology slides Performing frozen sections Performing autopsies Initial to be done at VA Medical Center, specimens read at Contractor's Lab Provide peer review for surgical reports Provide at Contractor's Lab Provide peer review for cytology reports Callback hours for emergent procedures Provide Cytotechnologist for CT guided biopsies Additional pathology services include: Participate in monthly Tumor Board meetings at VA Medical Center.. Be an active member of Invasive Procedures Committee (Quarterly meeting at VA Medical Center). Be an active member of Infection Control Committee (Quarterly meeting at VA Medical Center). Participate in continuing education and equipment acquisition planning throughout Laboratory Service at the VA Medical Center. Participate in quality control and quality assurance activities of the Pathology Department to comply with JCAHO and CAP requirements. Be present at the VA Medical Center for CAP and JCAHA Reviews. Laboratory services will include processing of surgical, autopsy and cytology specimens, reading of cytology slides. In addition, cytology containers with preservative are to be supplied by the contractor, as well as, disposal of 10% formalin and excess tissue. All of these services are to be provided at the Contractor's Lab. Surgical Specimens Contractor to provide all supplies: cassettes, inks and decalcification solution Autopsied tissue Cytology specimens/year: a. Sputum b. Urine c. PAP d. FNA Includes 4 slides e. Body Fluid f. Bronchoscopy Includes 4 slides g. Body Fluid Cell Blocks h. FNA - The extended study (more than 4 slides) i. Bronchoscopy - The extended study (more than 4 slides) Disposal of 10% formalin and excess tissue Decalification Procedure Special Stain Group I Special Stain Group II Immunoperoxidase Stain The total contract period shall be one base year with one year option periods not to exceed four additional years. Selection criteria include in descending order importance: (1) technical experience; (2) professional qualifications; (3) past experience working on VA facilities and/or Government; (4) geographical location of the firm within a 100 mile radius of the VA Medical Center, Grand Junction, Colorado; and (5) ability of the firm to begin the work within 15 days of award of the contract. All interested firms who can meet the requirements stated above should respond, in writing, with information describing their interest, capabilities, geographic location and indicate which small business category for which they qualify to the Contracting Officer shown above no later than 4:00 p.m. (Mountain Time), Wednesday, Jul 2, 2008. Solicitation VA-259-08-RP-0209 has been assigned and should be referenced on all correspondence regarding this announcement. THIS IS NOT A REQUEST FOR PROPOSAL. The North American Industrial Classification System (NAICS) number is 621511 and the business standard is $11.5M. is applicable to this acquisition. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database prior to award of a contract.
