Description
The Federal Bureau of Investigation (FBI) has a requirement to provide complete physical examinations, utilizing board-certified physicians at the following locations: New York, Newark,Boston, Philadelphia, and Los Angeles. The requirements include the following arears: (1) Board-Certified Physicians in any of the following specialties: cardiology, emergency medicine, internal medicine, family medicine, or preventive medicine (aerospace, occupational or general preventive medicine and public health), (2) Complete Medical History, (3) Hands-on Examinations, (4) Physical Examinations with 8 different sub-types, (5) Documentation and Findings, and (6) Laboratory Tests. A required Site Visit is part of the evaluation process. Additionally, all proposed physicians shall complete a Standard Form 86 Questionnarie for National Security Positions. All proposed medical personnel shall be subject to a background check, to be performed by the FBI. The Government intends to awards Indefinite Delivery Indefinity Quantity, Firm-Fixed price contracts for each location. The resultant contract will have a base year and four one-year option periods. The FBI reserves the right to make multiple awards. The solicitation package is available upon request. Offerors shall submit past performance information in their proposal. At a minimum, this shall consist of at least three most recent contract actions and include customer points of contacts with telephone numbers. In addition to past performance and, the proposed facility, Technical qualifications and Price will be evaluated. The order of importance will be stated in the solicitation document. All vendors must be registered in the Central Contractor Registration Database at www.ccr.gov prior to receiving an award. Interested parties are encouraged to review the website: www.FebBizOpps.gov for changes to the solicitation in the interim time until award of this requirement. Quotations will be due fifteen (15) days after the release of the solicitation. Telephone requests will not be honored.
